(1) Background: Controversial conclusions have been made in previous studies regarding the influence of autologous platelet-rich plasma (PRP) reproductive outcomes women with repeated implantation failures (RIF) who are undergoing embryo transfer (ET). (2) Methods: This study aimed to evaluate effect PRP intrauterine infusion patients unexplained RIF, vitro fertilization (IVF) or intracytoplasmic injection (ICSI), by a systematic review and meta-analysis. (3) Results: A fixed-effects model was used, 795 cases 834 controls were included these studies. The pooling results showed beneficial which compared those control terms clinical pregnancy rates (n = 10, risk ratio (RR) 1.79, 95% confidence intervals (CI): 1.55, 2.06; p < 0.01, I2 40%), live birth 4, RR 2.92, CI: 2.22, 3.85; 83%), 3, 1.74, 1.34, 2.26; 0%), positive serum β-HCG 14 days after ET 8, 1.77, 1.54, 2.03; 36%). However, we did not find that miscarriage indicated significant difference between two groups 6, 1.04, 0.72, 1.51; 0.83, 0%). (4) Conclusions: findings this systemic meta-analysis suggest appears improve IVF/ICSI treatments RIF.

Implantation success relies on intricate interplay between the developing embryo and maternal endometrium. Extracellular vesicles (EVs) represent an important player of this intercellular signalling through delivery functional cargo (proteins RNAs) that reprogram target cells protein RNA landscape. Functionally, reciprocity endometrial EVs regulates site implantation, preimplantation development hatching, antioxidative activity, attachment, trophoblast invasion, arterial remodelling, immune tolerance. Omics technologies including mass spectrometry have been instrumental in dissecting EV regulate these processes as well molecular changes endometrium to facilitate implantation. This has also led discovery potential human uterine fluid (UF) spent media (ESM) diagnostic therapeutic value implantation success, fertility, pregnancy outcome. review discusses contribution hallmarks how integration various omics is enabling design EV-based platforms reproductive medicine.

Despite the advances in field of reproductive medicine, implantation failure represents a challenging condition affecting 10–30% patients subjected to vitro fertilization (IVF). Research has focused on identification molecules playing crucial roles endometrial receptivity, with aim designing predictive tools for efficient detection window. To that end, novel molecular genomic and transcriptomic approaches have been introduced as promising enable personalized optimizing embryo transfer dating. However, clinical value these remains unclear. The this study is provide systematic review critical analysis existing evidence regarding employment commercially available evaluate receptivity. An Embase PubMed/Medline search was performed 1 February 2022. From 475 articles yielded, only 27 were included analyzed. considerable heterogeneity indicates uniqueness window, showcasing optimal time varies significantly between women. Moreover, provides information technical aspects advanced tools, well an possible biomarkers providing basis future research field.

Abstract The existence of Lactobacillus-led colonized bacteria in the endometrium a healthy human has been reported recent studies. Unlike composition microbiome lower genital tract, that is different and closely associated with physiological pathological processes gynecological diseases. For example, changing immune microenvironment affects receptivity endometrium, thereby leading to abnormal reproductive outcomes, such as embryo implantation failure recurrent spontaneous abortion. However, concrete functions mechanisms endometrial have not studied thoroughly. This review elaborates research progress on by which from perspective regulation. Considering lack unified evaluation method for microbiome, well an optimal treatment protocol against abortion, we also discussed application combining antibiotics probiotics/prebiotics precautionary measures.

Abstract Background Patients with recurrent implantation failure (RIF) may have more uterine contractions. Several observational studies suggested that atosiban administration around embryo transfer resulted in higher pregnancy rates RIF patients. This study aimed to evaluate the effect of given before fresh on outcomes women RIF. Methods A prospective, randomized, double-blind controlled clinical trial was performed IVF center Shanghai First Maternity and Infant Hospital. According a computer-generated randomization list, 194 infertile received between July 2017 December 2019 were randomly allocated into ( n = 97) placebo groups. Women treatment group intravenously about 30 min bolus dose 6.75 mg over one minute. Those only normal saline infusion for same duration. Results There no significant difference live birth rate groups (42.3% vs 35.1%, P 0.302, RR 1.206 (0.844–1.723)). No differences found two positive test, pregnancy, ongoing miscarriage, multiple ectopic rates. Similar results when stratified by number embryos previously transferred, previous failed transfers, frequency endometrial peristalsis day (≥ 3 waves/min) or serum estradiol (E 2 ) hCG above median level. And, there correlation E level day. The day, total FSH/HMG dosage duration factors which independently predicted likelihood birth. Conclusions These might not improve Trial registration had been approved Institutional Review Board hospital (2017 ethics No.43) registered under Clinicaltrials.gov an identifier NCT02893722.
