Background Podocyte injury is a common pathologic mechanism in diabetic kidney disease (DKD) and obesity-related glomerulopathy (ORG). Our previous study confirmed that Inonotus obliquus (IO) improved podocyte on DKD rats. The current explored the pharmacological effects, related mechanisms possible active components of IO ORG mice. Methods Firstly, by combining ultra-high performance liquid chromatography tandem mass spectrometry analysis (UPLC-Q-TOF-MS) with network pharmacology to construct human protein-protein interaction enrich pathway, which led discover crucial against ORG. Then, mice were established high-fat diet biochemical assays, histopathology, Western blot used explore effects obesity injury. Finally, pharmacology-based findings immunohistochemistry. compositions absorbed plasma analyzed UPLC-Q-TOF-MS molecular docking was predict compounds. Results result suggested alleviated inflammatory response modulating TNF signal. 20-week vivo experiment glomerular hypertrophy, under electron microscopy, renal nephrin, synaptopodin, TNF-α IL-6 expressions blotting immunohistochemical staining. Other indicators such as body weight, serum total cholesterol, liver triglyceride also intervention. showed triterpenoids, including inoterpene F trametenolic acid, might be pharmacodynamic basis. Conclusion research based analysis, amelioration via its modulation Triterpenoids predicated acting components.

Objectives Diabetic kidney disease (DKD) is driven by mitochondrial dysfunction and immune dysregulation, yet the mechanistic interplay remains poorly defined. This study aimed to identify key molecular networks linking pathways DKD progression, with a focus on uncovering biomarkers therapeutic targets.

Abstract Aim To ascertain the connection between normal‐range urinary albumin‐to‐creatinine ratio (UACR) and all‐cause mortality (ACM) among diabetic adults with preserved eGFR. Methods We used data from 2003–2018 National Health Nutrition Examination Survey. Nationally representative cross‐sectional survey linked outcomes Death Index. Restricted cubic spline curves (RCS) multivariable Cox regression models alongside subgroup analyses were utilised for estimating hazard ratios (HRs) 95% confidence intervals (Cls) UACR‐ACM interplay, adjusting demographic, socioeconomic, biochemical, medication medical history factors. The UACR's predictive accuracy survival was determined through receiver operating characteristic analysis. Results RCS analysis showcased that there no significant evidence to support a nonlinear relationship UACR ACM ( p = 0.080 nonlinearity) in participants diabetes mellitus (DM). In model 2 adjusted multiple confounding variables, HR 1.22 (95% CI, 1.06–1.40) per 10 mg/g raise continuous 1.50 (95%CI, 1.18–1.91) high tertile compared low. Kaplan–Meier showed significantly lower rates medium groups < 0.001). Subgroup manifested UACR–body mass index (BMI) interaction 0.033 interaction). Conclusions DM without overt kidney dysfunction, elevated independently related escalated ACM, particularly those normal BMI. conclude, we underscore significance of early risk assessment patients albuminuria, even dysfunction.
