Infertility is a major medical and social issue. In vitro fertilization an advanced ART technique that helps to successfully cure infertility. Quality preparation for IVF protocol the subsequent pregnancy important should include treatment of concomitant pathology, evaluation condition uterine cavity its optimal in order prevent reproductive failures early obstetric losses. This study assessed effectiveness optimized cryoprotocols embryo transfer patients with infertility comorbid course leiomyoma chronic endometritis. We performed clinical examination comparative analysis anamnestic data 100 women The were divided into two groups depending on proposed scheme outcomes compared control group. all groups, stimulation superovulation was carried out following short GnRH antagonist. determined endometritis, when group patients, had history menarche, onset sexual life, greater number partners, incidence urogenital infections, intrauterine surgical procedures. modified received lower mean total dose gonadotropins, shorter duration stimulation, fewer days anti-GnRH administration traditional also higher frequency delivery those conventional treatment. An appropriate anti-inflammatory antibiotic therapy comprehensive are key factors success protocols.

Abstract BACKGROUND The establishment and maintenance of pregnancy depend on endometrial competence. Asherman syndrome (AS) intrauterine adhesions (IUA), or atrophy (EA) thin endometrium (TE), can either originate autonomously arise as a result from conditions (i.e. endometritis congenital hypoplasia), medical interventions (e.g. surgeries, hormonal therapies, uterine curettage radiotherapy). Affected patients may present an altered inadequate lining that hinders embryo implantation increases the risk poor outcomes miscarriage. In humans, AS/IUA EA/TE are mainly treated with surgeries pharmacotherapy, however reported efficacy these therapeutic approaches remains unclear. Thus, novel regenerative techniques utilizing stem cells, growth factors, tissue engineering have emerged to improve reproductive outcomes. OBJECTIVE AND RATIONALE This review comprehensively summarizes methodologies emerging biotechnologies (cellular, acellular, bioengineering approaches) treat human pathologies. Regenerative therapies derived tissues blood which were studied in preclinical models (in vitro vivo) clinical trials discussed. SEARCH METHODS A systematic search full-text articles available PubMed Embase was conducted identify original peer-reviewed studies published English between January 2000 September 2023. terms included: human, uterus, endometrium, syndrome, adhesions, atrophy, endometritis, hypoplasia, curettage, radiotherapy, therapy, bioengineering, vesicles, platelet-rich plasma, biomaterials, microfluidic, bioprinting, organoids, hydrogel, scaffold, sheet, miRNA, sildenafil, nitroglycerine, aspirin, hormone, progesterone, estrogen. Preclinical cellular, strategies repair regenerate included. Additional identified through manual searches. OUTCOMES From total 4366 records identified, 164 (3.8%) included for review. Due heterogeneity study design measured outcome parameters both studies, findings evaluated qualitatively quantitatively without meta-analysis. Groups using cell-based treatments pathologies commonly employed mesenchymal cells (MSCs) bone marrow umbilical cord. Alternatively, acellular based plasma (PRP) extracellular vesicles gaining popularity. These accompanied by emergence matrix (ECM)-derived hydrogels synthetic biosimilars sustain local delivery reporting promising results. Combined target multiple aspects regeneration remain testing but shown translational value. highlights myriad material sources, administration methods, carriers been tested. WIDER IMPLICATIONS Therapies promote proliferation, vascular development, help restore function and, ultimately, fertility. Based existing evidence, cost, accessibility, availability we propose development triple-hit strategies, potentially combining high-yield MSCs cord) (PRP), possibly integrated ECM hydrogels. Advances together insights will pave way developing personalized treatment regimens infertility-causing disorders such AS/IUA, EA/TE, endometritis. REGISTRATION NUMBER https://osf.io/th8yf/

Chronic endometritis (CE) is the persistent inflammation of endometrial lining associated with infertility and various forms reproductive failures. The diagnosis CE based on histological evidence stromal plasma cells; however, standardized methods to assess cells are still lacking. In present paper, we aimed determine most appropriate cell threshold diagnose pregnancy outcomes. Three electronic databases were searched from their inception February 2022 for all studies comparing outcomes between patients without CE. relative risk (RR) pregnancy, miscarriage, and/or live birth rates calculated pooled adopted. A p-value < 0.05 was considered significant. Nine adopting different thresholds (1 50 cells/10 HPF) included. meta-analysis, only found a significant association miscarriage rate count ≥ 5/10 HPF (RR = 2.4; p 0.007). Among not suitable showed an worsened when high (10 50/10 conclusion, our study suggests that presence at low levels (<5/10 may predict Based these findings, ≥5 be more

Chronic endometritis (CE) is an inflammatory pathologic condition of the uterine mucosa characterized by unusual infiltration CD138(+) endometrial stromal plasmacytes (ESPCs). CE often identified in infertile women with unexplained etiology, tubal factors, endometriosis, repeated implantation failure, and recurrent pregnancy loss. Diagnosis has traditionally relied on biopsy histopathologic/immunohistochemistrical detection ESPCs. Endometrial biopsy, however, a somewhat painful procedure for subjects does not allow us to grasp whole picture this mucosal tissue. Meanwhile, fluid hysteroscopy been recently adopted as less-invasive diagnostic modality CE. We launched ARCHIPELAGO (ARChival Hysteroscopic Image-based Prediction histopathologic chronic Endometritis using deep LeArninG mOdel) study construct hysteroscopic finding-based prediction tools The development these learning-based novel models computer-aided detection/diagnosis systems potentially benefits suffering from elusive disease.

Abstract Background The aim of this retrospective study was to investigate whether oral antibiotics (doxycycline and metronidazole) combined with intrauterine perfusion (gentamicin dexamethasone) are beneficial for patients repeated implantation failure (RIF) chronic endometritis (CE) improve clinical pregnancy outcomes. Methods Patients RIF CE were diagnosed using hysteroscopy histology together. A total 42 enrolled in the study. All received 22 underwent immediately after end antibiotic therapy. Pregnancy outcomes evaluated during first vitro fertilization (IVF) embryo transfer (ET) cycle. Results For D3 ET treatment dexamethasone), higher rate (30.95% vs. 26.67%, P = 0.0308), (30% 50%, < 0.001), live birth (33.33% 45.45%, 0.0001). No fetal malformations or ectopic pregnancies observed. Conclusion We report as a novel successful compared those alone.

Endometriosis (EMS) and chronic endometritis (CE) frequently coexist. This study aimed to examine the impact of CE on reproductive outcomes in patients with EMS. We enrolled 685 endometriosis-associated infertility from January 2018 December 2020. The were divided into (318) non-CE (367) groups. A subset 123 clinically pregnant women group 369 was selected for detailed comparison. Pregnancy delivery data meticulously collected hospital records through telephone interviews. diagnosed 46.42% EMS patients. Higher pregnancy rates observed Fertility Index (EFI) scores 7–10. had increased risks placenta previa (13.01%), gestational hypertension (5.69%), cesarean sections (59.34%). CE, which is prevalent among patients, linked complications, including previa, hypertension, delivery. Although combined hysteroscopy laparoscopy improve rates, they demand careful management these complications.

Abstract Background: Endometriosis (EMS) and chronic endometritis (CE) often coexist. Laparoscopic hysteroscopic surgery is one of the main methods for diagnosing treating these conditions. The objectives this study were to examine clinical characteristics prognosis surgically diagnosed EMS complicated by CE assess reproductive outcomes during subsequent pregnancies. Methods:A total 1574 infertile patients included in between January 2018 December 2020. 685 with endometriosis who further divided into two groups: a group consisting 318 non-CE 367 patients. Within cohort suffered from CE, subset postoperative care pregnant women (n=123) was selected form group. control consisted undergoing singleton pregnancy (n=369) underwent delivery same period. Data on statuses complications collected hospitals through telephone interviews. Results: prevalence rate 46.42%. Among posterior 135 achieved pregnancy. cumulative found be significantly higher groups fertility index (EFI) scores ranging 7 8 9 10 compared EFI below 7. Furthermore, both had risk experiencing pregnancy, including placenta previa (13.01%), gestational hypertension (5.69%), cesarean section (59.34%). Conclusions:Patients exhibited heightened endometritis. Our investigation revealed that combined laparoscopic proved successful approach augmenting overall rate. afflicted face an independently escalated susceptibility previa, hypertension, delivery.
