Abstract Background : Follicular thyroid carcinoma (FTC) is a differentiated malignancy that exhibits the ability to metastasize hematogenously distant organs, including lungs and bones. Spinal metastasis an unusual site for even when occurs, presents late in disease course often associated with advanced poor prognosis. Until 2019, literature only showed 29 cases of FTC spinal as first presenting feature. Case Presentation We present case 67-year-old female no previous history who presented acute onset severe neurological deficit ended up bedridden. Magnetic resonance imaging (MRI) spine revealed lesion causing cord compression. Urgent surgical decompression was performed, histopathological analysis confirmed presence metastatic FTC. Subsequent comprehensive evaluation, ultrasonography unveiled very small primary tumor. Conclusion This report highlights importance considering differential diagnosis patients metastasis, absence known malignancy. Prompt diagnosis, staging, multidisciplinary management are crucial optimizing treatment outcomes improvement

A 64-year-old female was referred to our General Surgery clinic with a posterior scalp lump. She had presented her GP, lump in occipital area of about 3 cm diameter, which gradually increasing size.
