Abstract Background and Objective Mendelian randomization (MR) has become an important tool in epidemiology, used to infer causal relationships diseases. This review aims consolidate existing MR evidence concerning celiac disease (CeD). Methods We systematically searched major databases up May 2024, adhering PRISMA guidelines. Only studies explicitly investigating CeD were included. assessed the quality of each study based on strength, independence, exclusivity instrumental variables used. Results From initial pool 207 articles, 35 met our inclusion criteria. These frequently addressed relationship between autoimmune diseases like inflammatory bowel (IBD) explored connections with gut microbiota, various cancers, metabolic disorders. Significant findings highlight a robust bi-directional association IBD complex interactions microbiota. Notably, many associations reported near threshold clinical significance. Conclusion systematic highlights dual nature current CeD. On one hand, we observe consistent CeD, IBDs, other, there is plethora weaker that raise critical questions about their research work lays solid foundation for deeper investigations into these links, particularly relation lymphomas psychiatric conditions. It calls expanded use other methodologies explore under-researched areas.
