ABSTRACT Background Spinal cord injury (SCI) is a complex central nervous system disorder characterized by multifaceted pathological processes, including inflammation, oxidative stress, programmed cell death, autophagy, and mitochondrial dysfunction. Sirtuin 1 (Sirt1), critical NAD + ‐dependent deacetylase, has emerged as promising therapeutic target for SCI repair due to its potential protect neurons, regulate glial vascular cells, optimize the microenvironment. However, regulatory roles of Sirt1 in are challenging, effects vary depending on activation timing, expression levels, types. Methods A systematic literature review was conducted using PubMed, Scopus, Web Science identify studies investigating SCI. Relevant publications were analyzed synthesize current evidence Sirt1's mechanisms, effects, challenges repair. Results exerts broad across diverse processes types post‐SCI. It promotes neuronal survival axonal regeneration, modulates astrocytes microglia resolve supports oligodendrocyte‐mediated myelination, enhances endothelial function. Proper may mitigate secondary injury, whereas excessive or prolonged could impair inflammatory resolution disrupt cellular homeostasis. This highlights therapies, but include optimizing spatiotemporal addressing dual different Conclusion Targeting represents viable strategy repair, given regulation neuroprotection, immunomodulation, tissue remodeling. translating these findings into therapies requires resolving issues such type‐specific delivery, precise dosage control. provides theoretical foundation practical insights advancing Sirt1‐based treatments

Tissue resident regulatory T cells (tissue Tregs) are vital for maintaining immune homeostasis and controlling inflammation. They aid in repairing damaged tissues influencing the progression of fibrosis. However, despite extensive research on how tissue Tregs interact with non-immune during repair, their pro- anti-fibrotic effects chronic injury remain unclear. Understanding various cell types, as well roles fibrosis, is crucial uncovering mechanisms behind these conditions. In this review, we describe repair fibrosis across different explore potential strategies regulating homeostasis. These insights hold promise providing new perspectives approaches treatment irreversible fibrotic diseases.

The acute phase of secondary spinal cord injury (SCI) is a crucial therapeutic window to mitigate ongoing damage and promote tissue repair. present systematic review critically evaluates the efficacy safety current management modalities for this phase, identifying gaps in knowledge providing insights future research directions. In December 2024, PubMed, Web Science, Google Scholar, Embase were accessed with no time constraints. All clinical studies investigating pharmacological SCI accessed. Data from 3017 patients (385 women) collected. mean length follow-up was 6 ± 3.4 months, age 43.3 10.3 years. Erythropoietin (EPO) improves motor function, reduces impairment injury, modulates antioxidation neurogenesis, minimizes apoptosis inflammation. Although commonly administered, methylprednisolone shows uncertain efficacy. rho-GTPases inhibitor VX-210 levetiracetam did not demonstrate effectiveness treatment. Monosialotetrahexosylganglioside Sodium Salt (GM-1) riluzole are associated favorable neurological outcomes. Granulocyte Colony-Stimulating Factor (G-CSF) Hepatocyte Growth (HGF) offer improved scores fewer side effects.

Post-operative cognitive decline (POCD) is characterized by impairments in functions. Coronary artery bypass grafting (CABG) associated with a high risk of POCD due to its impact on neuroinflammation and oxidative stress. In this study, we investigated the dynamics neurotrophic, inflammatory, stress markers cohort post-CABG patients identify potential biomarkers for POCD. Blood samples were collected at baseline (immediately post-surgery) 3-month follow-up. Expression levels NRF2 other regulators (GST, GSS, HMOX1, CAT, HSP27, LOX-1), inflammatory mediators (IL-6, IP-10, NFκB), neuroprotective factor (BDNF) analyzed. Cognitive assessments performed using RBANS, TMT, TIB MMSE. exhibited an initial upregulation NRF2-related antioxidant genes, which failed sustain 3-months follow-up, leading IP-10 BDNF protein levels, along increased LOX-1 level NFκB expression, indicating persistent inflammation. contrast, non-POCD demonstrated sustained increase markers, suggesting more effective compensatory response. ROC analysis identified HMOX1 as significant predictors POCD, emerging diagnostic conclusion, our findings highlight dynamic regulation pathways emphasizing failure neuroprotection affected patients. Further large-scale studies are necessary validate these findings, biomarker-based screening could facilitate early stratification targeted interventions improve outcomes after cardiac surgery.

Artemisinin and its derivatives are widely recognized as first-line treatments for malaria worldwide. Recent studies have demonstrated that artemisinin-based antimalarial drugs, such artesunate, dihydroartemisinin, artemether, not only possess excellent properties but also exhibit antitumor, antifungal, immunomodulatory effects. Researchers globally synthesized artemisinin like SM735, SM905, SM934, which offer advantages low toxicity, high bioavailability, potential immunosuppressive properties. These compounds induce immunosuppression by inhibiting the activation of pathogenic T cells, suppressing B cell antibody production, enhancing differentiation regulatory cells. This review summarized mechanisms analogs modulate excessive inflammation immune responses in rheumatic skeletal diseases, autoimmune inflammatory disorders, through pathways including TNF, Toll-like receptors, IL-6, RANKL, MAPK, PI3K/AKT/mTOR, JAK/STAT, NRF2/GPX4. Notably, context NF-κB pathway, inhibits expression disrupting upstream cascades and/or directly binding to downregulates multiple downstream genes controlled NF-κB, chemokines their receptors. targets regulate various functions, apoptosis, proliferation, signal transduction, antioxidant responses, ultimately intervening systemic diseases organs kidneys, nervous system, skin, liver, biliary system modulating dysregulation responses. Ongoing multicenter randomized clinical trials investigating effects these on rheumatic, inflammatory, with aim translating promising preclinical data into applications.

Spinal cord injury (SCI) is a severe medical condition resulting in substantial physiological and functional consequences for the individual. People with SCI are characterised by chronic, low-grade systemic inflammatory state, which contributes to further undesirable secondary injuries. This study aimed evaluate effect of adding aquatic therapy standard physiotherapy treatment, implemented two different schedules, on inflammation patients. Additionally, relationship between cytokine blood levels changes functionality (measured 6MWT, 10MWT, WISCI, BBS, TUG tests) throughout was assessed. A quantitative multiplexed antibody assay performed measure expression level 20 pro- anti-inflammatory cytokines samples from patients at three time points: baseline, week 6, immediately post-intervention (week 12). identified complex signature five (IL-12p70, IL-8, MCP-1, IL-1α, IP10) associated course programs. Two other (IL-4 TNF-α) were also recovery These could be important indicators prognosis provide basis developing novel targeted therapies.
