Abstract Purpose of Review In order to better treat pain, we must understand its architecture and pathways. Many modulatory approaches pain management strategies are only poorly understood. This review aims provide a theoretical framework perception modulation in assist clinical understanding research analgesia anesthesia. Recent Findings Limitations traditional models for have driven the application new data analysis models. The Bayesian principle predictive coding has found increasing neuroscientific research, providing promising background principles consciousness perception. It can be applied subjective pain. Summary Pain viewed as continuous hierarchical process bottom-up sensory inputs colliding with top-down modulations prior experiences, involving multiple cortical subcortical hubs matrix. Abstract The experience of pain is a combined product bottom-up and top-down influences mediated by attentional emotional factors. Acupuncture is a medical tool in which sterile needle used to penetrate and stimulate certain body area (acupoint), inducing series of sensations such as numbness, dullness, or aching, often referred de-qi. But that all? In this article, we adopt Bayesian perspective explore the cognitive affective aspects acupuncture beyond needling, specifically, how integrates bottom-up sensory signals with top-down predictions perception. We propose way discern treatment result predictive coding, probabilistic, inferential process our brain. Active inference from both prior experience expectations acupuncture, when integrated incoming signals, creates unique, individual internal generative model perception acupuncture. A framework coding may, therefore, aid elucidating quantifying components facilitate understanding their differential interactions determining treatment. Thus, perception-based presented article may expand on perceive treatment, simply inserting needles into one encompasses complex healing supported by belief hope regaining health. By exploring factors influence responsiveness review sheds light why more effective some individuals than others.

Abstract When do we first experience pain? To address this question, need to know how the developing nervous system processes potential or real tissue‐damaging stimuli in early life. In newborn, nociception preserves life through reflex avoidance of tissue damage and engagement parental help. Importantly, also forms starting point for experiencing learning about pain setting level adult sensitivity. The ability to perceive pain presents an interesting evolutionary advantage adapt ever-changing environment. However, in the case of chronic (CP), perception hinders capacity system changing sensory environments. Similar other perceptual disorders, CP is also proposed be a maladaptive compensation aberrant predictive processing. local-global oddball paradigm relies on learning hierarchical rules and processing environmental irregularities at local global level. Prediction errors (PE) between actual predicted input typically trigger update forward model limit probability encountering future PEs. It has been hypothesised that updating, reflected increased deviance decreased deviance. In present study, we used examine how influences relative healthy controls. As hypothesised, observed stimulus characteristics evoked heightened stimulus-driven PE. This accompanied by respective changes theta phase locking correlated with subjective perception. Changes deviant stimulus-driven-PE could explained dampened attention-related responses. Changing context auditory did not however show difference context-driven These findings suggest updating where constant presence disrupts non-nociceptive domains. Furthermore, hypothesise auditory-processing based biomarker identified here marker domain-general dysfunction confirmed research.
