In patients with inflammatory bowel diseases (IBD), functional complaints frequently persist after the clearing of inflammation and are clinically difficult to distinguish from symptoms inflammation. recent years, influence bidirectional communication between gut brain on physiology, emotions, behavior has been demonstrated. What mechanisms underlie development gastrointestinal in irritable syndrome (IBS) IBD? therapeutic approaches arise this? Narrative review. The pathogenesis IBS involves interactions psychosocial factors, genetics, microbiota as well central peripheral nervous systems. interplay stress visceral hypersensitivity is importance. Therapeutically, lifestyle changes reduction exercise alongside dietary, pharmacological, psychotherapeutic options useful. treatment disorders remains challenging, pharmacological therapies often ineffective gut-directed psychotherapies rarely available.

ABSTRACT Introduction Crohn's disease (CD) is a chronic inflammatory condition associated with psychological stress and anxiety. Functional magnetic resonance imaging (fMRI) studies have shown differences in brain function between patients CD healthy controls (HC). This study aimed to compare the neural correlates of anxiety inindividuals relative HC, using resting‐state fMRI data. Methods Participants filled State–Trait Anxiety Inventory (STAI), validated tool for measuring anxiety, underwent an MRI acquisition, including both structural functional sequences, identify regions scores. Results Seventeen eighteen HC matched age, education, sex participated study. No significant group emerged STAI However, analysis revealed distinct patterns connectivity scores two groups. Among group, greater correlated increased connectivity, whereas, they decreased connectivity. Significant clusters were found belonging specific networks (RSNs): (a) Posterior Cingulate Cortex (PCC, within Default Mode Network), (b) left Middle Frontal Gyrus (within Left Fronto‐Parietal (c) PCC right Superior Temporal Dorsal Attention Network). Conclusion The differential association observed participants was located areas self‐referential (Default Network) cognitive (Left Network RSNs. Our findings suggest that maladaptive/dysfunctional processing negative emotions visceral sensitivity may occur CD.
