Abstract The identification of acute cardioprotective strategies against myocardial ischemia/reperfusion (I/R) injury that can be applied in the catheterization room is currently an unmet clinical need and several interventions evaluated past at pre-clinical level have failed translation. Autonomic imbalance, sustained by abnormal afferent signalling, a key component I/R injury. Accordingly, there strong rationale for neuromodulation strategies, aimed reducing sympathetic activity and/or increasing vagal tone, this setting. In review we focus on cervical nerve stimulation (cVNS) transcutaneous auricular vagus (taVNS); latest has potential to overcome issues invasive cVNS, including possibility being used setting, while retaining its beneficial effects. First, discuss pathophysiology injury, mostly consequence overproduction reactive oxygen species. Second, describe functional anatomy parasympathetic branch autonomic nervous system most relevant principles bioelectronic medicine electrical modulation, with particular taVNS. Then, provide detailed comprehensive summary studies non-invasive VNS support effect whenever or chronic cardiac specifically setting benefit emerging field post arrest syndrome (PCAS) also mentioned. Indeed, cVNS anti-adrenergic, anti-inflammatory, antioxidants, anti-apoptotic pro-angiogenic effect; involved molecular pathways were already directly confirmed take place Pre-clinical data clearly show sooner applied, better outcome, marked infarct size reduction almost complete left ventricular reverse remodelling when immediately before during reperfusion. Finally, detail limited but very promising experience taVNS available so far.

Neuromodulation comes into focus as a non-pharmacological therapy with the vagus nerve modulation target. The auricular stimulation (aVNS) has emerged to treat chronic diseases while re-establishing sympathovagal balance and activating parasympathetic anti-inflammatory pathways. aVNS leads still over under-stimulation is limited in therapeutic efficiency. A potential avenue personalization of based on time-varying cardiorespiratory rhythms human body. In pilot study, we propose personalized cardiac-gated evaluate its effects instantaneous beat-to-beat intervals (RR intervals). Modulation RR expected reveal efficiency since efferent cardiac branch stimulated afferent governs RR. Five healthy subjects were subjected aVNS. Each subject underwent two 25-min sessions. first session started non-gated open-loop aVNS, followed by systole-gated closed-loop then non-gated, diastole-gated, each for 5min. second session, systole diastole gated interchanged. Changes are analysed comparing prolongation respect proceeding interval where took place. These changes considered function onset, angle starting R peak. influence respiration phases cardiovagal modulation. results show that tends prolong shorten when after before peak, respectively. later time onset within diastole-gated longer be subsequent interval. tendency raises increasing gradually levels off delay from one slope this rise larger than When considering individual phases, inspiratory seems largest values thus modulatory capacity time-gated This study indicates modulate heartbeat activity which attenuated diseases. highest during inspiration.

Abstract The convergence of medical and technological developments has continued to transform the delivery care in disaster environments, incorporating advances from telecommunications physiologic monitoring, artificial intelligence, computer vision. However, unless interconnected nature these is conceptualized with a proper framework, there risk overlooking applications, developing silos, limiting interoperability between innovations. To develop such this piece integrated review current literature, expert insights, global market trends propose 4 categories innovations: (1) Enabling Technologies, (2) Signal Acquisition, (3) Data Utilization, (4) Applications. Applications can be further subdivided into use cases: Disease Injury Surveillance Detection, Population Protection, Responder Management. Practitioners, policymakers, private sector counterparts utilize framework change their clinical practices, allocate funds stepwise fashion, or prioritize development projects, respectively.

Neurotransmission is a complex process with multiple levels of regulation that, when altered, can significantly impact mental health. Neurons in the adult brain release more than one transmitter and environmental stimuli change type neurons express. Changes express generate changes animal behavior. The ability to transmitters and/or switch them response likely evolved provide flexibility complexity neuronal circuit function an ever-changing environment. However, this adaptability become maladaptive generating behavioral alterations that are unfit for environment which lives or tasks it needs perform. Repeated exposure addictive substances induces long-lasting molecular synaptic changes, driving appearance behaviors result drug misuse addiction. Recent findings have shown way drugs abuse alter by inducing Here, we review evidence prolonged number expressing neuropeptide orexin, neuromodulator dopamine, inhibitory GABA. These show drug-induced plasticity conserved across species, belonging different classes chemicals induce same plasticity, only cause types they Importantly, contributes long-term negative effects consumption, can, some cases, be either prevented reversed alleviate these outcomes. Regional hyperactivity appears modulate stabilization expression, no longer observed activity normalized. Overall, underscore importance continuing investigate extent significance neurotransmitter exploring whether non-invasive strategies used reverse as means mitigate use.
