Objective Previous cross-sectional studies have demonstrated that the body roundness index (BRI) is associated with knee osteoarthritis (KOA). However, no longitudinal confirmed this association. This study aims to explore link between BRI and KOA risk in Chinese population through analysis evaluate its utility early diagnosis prediction. Methods utilizes data from China Health Retirement Longitudinal Study (CHARLS). A total of 7,318 participants who were followed 2015 2020 included. was calculated using physical examinations questionnaire data, categorized by quartiles. The relationship assessed multivariate weighted regression models trend tests, while subgroup sensitivity analyses conducted ensure robustness findings. Results After 5 years follow-up, 1,035 (14.14%) diagnosed KOA. Findings indicate a positive correlation ( HR = 1.08, 95% CI : 1.02–1.13, p 0.0039), an increasing across quartiles for 0.0033). Subgroup reveals association particularly strong among individuals aged 50–59, males, those living rural areas, without cardiovascular disease. Conclusion establishes increase significantly elevates risk. These findings suggest could be effective tool assessment contribute development personalized prevention strategies. Additionally, valuable elucidating potential mechanisms linking fat distribution inflammatory responses progression.
