Background/Objectives: Osteoarthritis (OA) is a prevalent and debilitating joint disorder characterized by progressive cartilage degradation inflammation, for which traditional treatments offer only symptomatic relief without halting disease progression. Exosomes, cell-free vesicles derived from mesenchymal stem cells, have emerged as promising alternative therapy owing to their regenerative anti-inflammatory properties. Methods: This review synthesizes findings recent studies (2017–2023) on the therapeutic potential of exosomes in OA treatment, highlighting ability modulate microenvironment, reduce promote repair delivering bioactive molecules such cytokines, growth factors, regulatory ribonucleic acids. Results: We explore integration with biomaterials, hydrogels scaffolds, enhance delivery efficacy, we address critical challenges associated clinical application, including standardization isolation characterization methods, scalability production, mechanistic understanding, long-term safety. Despite these challenges, exosome-based therapies several advantages over cell-based treatments, lower immunogenicity, ease handling, targeted agents damaged tissues. Conclusions: provide an analytical perspective current state exosome research OA, emphasizing need standardized production deeper insights, rigorous safety assessments. Future directions should focus optimizing systems, exploring personalized medicine approaches, conducting comparative effectiveness fully realize treatment. Addressing gaps will be crucial translating bench bedside achieving transformative impact management.
