Background Acute pulmonary embolism (APE) is classified as a subset of diseases that are characterized by lung obstruction due to various types emboli. Current clinical APE treatment using anticoagulants frequently accompanied high risk bleeding complications. Recombinant hirudin (R-hirudin) has been found have antithrombotic properties. However, the specific impact R-hirudin on remains unknown. Methods Sprague-Dawley (SD) rats were randomly assigned five groups, with thrombi injections establish models. Control and group subcutaneously injected equal amounts dimethyl sulfoxide (DMSO). The APE+R-hirudin low-dose, middle-dose, high-dose groups received subcutaneous at doses 0.25 mg/kg, 0.5 1.0 respectively. Each was subdivided into time points 2 h, 6 1 d, 4 animals per point. Subsequently, all euthanized, serum tissues collected. Following assessment right ventricular pressure (RVP) mean artery (mPAP), blood gas analysis, enzyme-linked immunosorbnent assay (ELISA), vascular testing, hematoxylin-eosin (HE) staining, Terminal deoxynucleotidyl transferase-mediated dUTP-biotin nick end labeling (TUNEL) immunohistochemistry, Western blot experiments conducted. Results caused significant reduction mPAP, RVP, Malondialdehyde (MDA) content, well H O myeloperoxidase (MPO) activity, while increasing oxygen (PaO ) Superoxide Dismutase (SOD) activity. also decreased wall area ratio thickness diameter in rat arteries. Serum levels endothelin-1 (ET-1) thromboxaneB2 (TXB2) decreased, prostaglandin (6-K-PGF1 α NO increased. Moreover, ameliorated histopathological injuries reduced apoptotic cells Matrix metalloproteinase-9 (MMP9), cell adhesion molecule-1 (VCAM-1), p-Extracellular signal-regulated kinase (ERK)1/2/ERK1/2, p-P65/P65 expression tissues. Conclusion attenuated hypertension thrombosis rats, suggesting its potential novel strategy for APE.
