This review explores the critical role of exosomes in promoting angiogenesis, a key factor skin flap survival. Skin flaps are widely used reconstructive surgery, and their survival depends heavily on formation new blood vessels. Exosomes, small extracellular vesicles secreted by various cells, have emerged as important mediators intercellular communication play crucial biological processes such angiogenesis. Compared to traditional methods show more selective targeted therapeutic potential they naturally carry angiogenic factors can precisely regulate angiogenesis process. Abstract Ischemic stroke, the second leading cause of human mortality, presents a formidable challenge to healthcare. Following ischemic insult, brain undergoes intricate pathological transformations, prominently marked by mitochondrial damage, including swelling, fission, and mitophagy, collectively termed quality control disorder. Mitochondria, pivotal in energy regulation oxidative stress modulation, play critical role neuronal apoptosis post‐stroke. To solve problems caused disorders, transfer has become new therapeutic strategy for central nervous system diseases. Mitochondrial refers process which certain cell types export their mitochondria pass them on other types, also known as intercellular transfer. Mechanistically, occurs via tunneling nanotubes, extracellular vesicles, free transfer, exerting multifaceted effects such anti‐inflammatory, anti‐lipid peroxidation, ferroptosis enhancement metabolism. This review explores efficacy, current obstacles, future prospects offering insights researchers instilling hope patients conquering this debilitating condition.
