We describe, to our knowledge, the first use in Dubai of extracorporeal life support (ECLS) a patient who suffered intraoperative cardiac arrest due presumed channelopathy. A 40-year-old presented for open myomectomy surgery. She had no other medical problems apart from obesity. The denied any family history surgery or anesthesia-related complications. Her initial electrocardiogram (ECG) and laboratory results were within reference limits. Intraoperatively, sudden arrest, which she was resuscitated. Point-of-care ultrasound intensive care unit (ICU)-performed echocardiography revealed severely reduced left ventricular contractility. Veno-arterial membrane oxygenation (V-A ECMO) intra-aortic balloon pump utilized immediate postoperative period. Although patient's heart sustained more than 300 DC shocks, it recovered. Family members that brother sister died their 20s death. Another been resuscitated few years ago too. case illustrates importance disclosure relevant information. It supports notion ECLS can be used period if surgical hemostasis is achieved. Controversies, such as heparinization risk bleeding, transthoracic (TTE) versus transesophageal (TEE), timely insertion intracardiac defibrillator, genetic screening, are discussed. learning point clinicians do not work vacuum. Organizational leadership greatly impact outcomes, creating conditions safer care.

Heart failure is a complex clinical syndrome characterized by the heart’s inability to pump sufficient blood meet body’s metabolic demands, resulting in symptoms such as dyspnea, fatigue, and fluid retention. Despite significant advances pharmacological treatments device therapies, prognosis for patients with advanced heart remains poor. This underscores urgent need innovative regenerative therapies aimed at restoring cardiac function improving quality of life suffering from this debilitating condition. article provides comprehensive overview pathophysiologic mechanisms underlying end-stage failure, which include myocardial systolic diastolic dysfunction, neuroendocrine activation, inflammation, oxidative stress. Collectively, these lead progressive deterioration structure function, culminating failure. The also examines differences between ischemic non-ischemic cardiomyopathies their implications treatment strategies prognosis. Furthermore, review explores application medicine focusing on its potential repair functional recovery. In field medicine, induced pluripotent stem cells derived cardiomyocytes are considered cutting-edge technologies repair. Induced can differentiate into cardiomyocytes, thereby offering new hope regeneration. However, current research faces several challenges, including maturity integration immunogenicity concerns, difficulties scaling up production applications. Additionally, ethical issues related cell technology, sourcing tumorigenic risks, must be carefully addressed. highlights technology treating while emphasizing complexities involved transitioning laboratory practice.

Cardiovascular disease remains the leading cause of death worldwide, yet despite massive investment in drug discovery, progress cardiovascular drugs from lab to clinic slow. It is a complex, costly pathway discovery and failure becomes more expensive as progresses along this pathway. The focus has begun shift optimisation vitro culture methodologies, not only because these must be undertaken are earlier on pathway, but also principles 3Rs have become embedded national international legislation regulation. Numerous studies shown myocyte cell behaviour much physiologically relevant 3D compared 2D culture, highlighting advantages using 3D-based models, whether microfluidic or otherwise, for preclinical screening. This review aims provide an overview challenges limitations traditional routes, successes field models discovery. focuses particular role biomimicry can play, around implementation within commercial

Injured human hearts are fibrotic, whereas zebrafish functionally regenerate following myocardial injury. The unique regeneration niche microenvironment has been extensively studied in hearts. However whether this can be extrapolated to humans remains unclear owing significant species differences. We xenografted induced pluripotent stem cell-derived cardiomyocytes (hiCMs) into the cardiac region of one-day post-fertilized embryos and established a xenograft model hiCMs. This used explore behavior hiCMs transplanted Fluctuations fluorescence intensity genetically encoded calcium indicator protein GCaMP indicated that donor were beating. analyzed synchronization + heart. found synchronous beating between host 40 % with GCaMP-hiPSCs. Our chimeric heart potential bridge capacity gap proming future applications.

A healthy young woman, age 26 without prior cardiac complications, experienced an out-of-hospital arrest caused by ventricular fibrillation (VF), which coincided with a fever. Comprehensive diagnostics including echo, CMR, exercise testing, and genetic sequencing, did not identify any potential cause. This led to the diagnosis of idiopathic VF installment implantable cardioverter defibrillator, six months later appropriately intervened another episode under conditions comparable first event. second diagnostic opinion concluded short-coupled Torsade de Pointes (scTdP), patient was started on verapamil treatment. From this patient, human induced pluripotent stem cell cardiomyocyte (hiPSC-CM) lines were generated study cellular electrophysiology. Without known pathogenic variation, no isogenic control line could be produced, therefore age- sex-matched hiPSC-CM used. Cellular electrophysiology studied in these cardiomyocytes using calcium- voltage sensitive fluorescent dyes measurements carried out at 37 °C 39 °C, mimic condition hyperthermia patient. mRNA expression electrophysiologically relevant genes analyzed underlying mechanism. Calcium transients measured physiological temperature indicated occurrence early after (EATs). Strikingly, incidence EATs further increased. Membrane data from also revealed shorter action potentials that, combined EATs, indicate premature release calcium during diastole, responsible for extrasystoles Gene profiles mainly downregulated but clearly aid unraveling mechanism behind EATs. Pharmacological screening performed evaluate treatment regimen determine While verapamil, dantrolene, flecainide decrease handling parameters affected indicating functionality drugs. patient-specific case electrophysiological phenotyping resulted hypothesis possible scTdP arrhythmias, accentuates applicability disease modeling phenotyping.
