Sepsis is a life-threatening organ dysfunction due to dysfunctional response infection. Delays in diagnosis have substantial impact on survival. Herein, blood samples from 586 in-house patients with suspected sepsis are used conjunction machine learning and cross-validation define six-gene expression signature of immune cell reprogramming, termed Sepset, predict clinical deterioration within the first 24 h (h) presentation. Prediction accuracy (~90% early intensive care unit (ICU) 70% emergency room patients) validated 3178 existing independent cohorts. A RT-PCR-based Sepset detection test shows 94% sensitivity 248 worsening sequential failure assessment scores h. stand-alone centrifugal microfluidic instrument that automates whole-blood classifier tested, showing 92%, specificity 89% identifying risk sepsis.
