Management of Oro-Antral Communication: A Systemic Review of Diagnostic and Therapeutic Strategies

Aim:
This
study
aims
to
evaluate
the
management
of
oro-antral
communications
(OAC)
and
fistulas
(OAF),
focusing
on
treatment
strategies
based
defect
size,
epithelialization,
presence
sinus
infections,
while
exploring
both
traditional
emerging
techniques.
Materials
Methods:
The
systematic
review
was
conducted
following
PRISMA
guidelines
registered
PROSPERO
(CDR
ID
623251).
Using
targeted
keywords,
articles
in
English
published
within
last
10
years
were
analyzed
from
databases
such
as
PubMed,
WoS
Scopus,
selecting
only
clinical
studies
human
patients.
After
thorough
screening,
20
publications
included
qualitative
analysis,
among
734
initially
identified.
Results:
Small
OACs
(<5
mm)
managed
conservatively
with
hemostatic
materials,
larger
defects
(>5
required
surgical
closure,
Bichat
flap
proving
highly
effective
for
large
defects.
Innovative
treatments
using
autologous
bone
grafts
PRF
showed
promise
supporting
tissue
regeneration.
In
cases
sinusitis,
combination
FESS
intra-oral
closure
techniques
resulted
high
success
rates
infection
resolution
closure.
Conclusions:
Treatment
outcomes
OAC
OAF
are
dependent
size
sinusitis.
Multidisciplinary
collaboration,
along
timely
intervention
adherence
medical
therapies,
is
essential
successful
management.
Emerging
minimally
invasive
procedures
continue
improve
patient
outcomes,
offering
hope
more
sustainable
solutions
complex
cases.

This
study
