Scientific publications grew significantly from 2.1 million in 2012 to 3.3 2022, with AI research tripling during this period. Artificial Intelligence (AI) is transforming pancreaticobiliary endoscopy by enhancing diagnostic accuracy, procedural efficiency, and clinical outcomes. This narrative review explores AI's applications in endoscopic ultrasound (EUS) retrograde cholangiopancreatography (ERCP), emphasizing its potential to address therapeutic challenges diseases. In EUS, AI improves pancreatic mass differentiation, malignancy prediction, landmark recognition, demonstrating high accuracy outperforming traditional guidelines. Despite advances in surgery, chemotherapy, and radiotherapy, the treatment of colorectal cancer (CRC) requires more personalized approaches based on tumor biology molecular profiling. While some relevant mutations have been associated with differential response to immunotherapy, such as RAS BRAF limiting anti-epithelial growth factor receptor drugs or microsatellite instability predisposing susceptibility immune checkpoint inhibitors, role inflammation dictating progression is still under investigation. Several inflammatory biomarkers identified guide patient prognosis. These include neutrophil-lymphocyte ratio, Glasgow prognostic score (GPS) its modified version, lymphocyte-C-reactive protein platelet-lymphocyte ratio.
