(1) Background: To investigate the correlation between integrity of left dual-stream frontotemporal network mediated by arcuate fasciculus (AF) and uncinate (UF), acute/subacute post-stroke aphasia (PSA). (2) Methods: Thirty-six patients were recruited received both a language assessment diffusion tensor imaging (DTI) scan. Correlations indices in bilateral LSAF/UF performance analyzed with analyses. Multiple linear regression analysis was also implemented to effects on performance. (3) Results: Correlation analyses showed that indices, including mean fractional anisotropy (FA) values fiber number LSAF rather than UF significantly positively associated domain scores (p < 0.05). revealed an independent positive association FA value percentage score subsets. In addition, no interaction effect found > (4) Conclusions: The LSAF, but not UF, might play important roles supporting residual ability individuals PSA; simultaneous disruption would result more severe damage either pathway alone.

Background: Intravenous thrombolysis and mechanical thrombectomy are the first-line reperfusion therapies for acute ischemic stroke. Here, we describe utility of diffusion magnetic resonance imaging (MRI) fiber tractography 123I-iomazenil benzodiazepine receptor single-photon emission computed tomography to estimate prognosis post-stroke aphasia after successful therapy. Case report: An 81-year-old man was admitted hospital approximately 3.5 h onset symptoms, including decreased consciousness, right hemiparesis, aphasia. MRI revealed cerebral infarction due M1 segment occlusion. alteplase followed by endovascular resulted in recanalization left middle artery territory. A subsequent showed no new or hemorrhagic lesions. Although patient’s motor hemiparesis gradually recovered, persisted. Diffusion performed 2 weeks admission partial injury arcuate fasciculus, indicated lower fractional anisotropy values than on contralateral side. density also detected perisylvian temporoparietal cortices. The patient clear signs further improvement chronic stage discharged a nursing home 3 months. Conclusions: application functional neuroimaging techniques assess neuronal damage primary brain regions therapy large-vessel occlusion may allow an accurate This have direct clinical implication navigating subacute-to-chronic phases rehabilitative care.

Aphemia is a rare and special type of speech disorder, the mechanisms underlying occurrence recovery remain unclear. Here, we present clinical case poor spontaneous aphemia, with anterior segment left arcuate fasciculus server damaged posterior intact, as detected by diffusion tensor imaging. could be caused disruption cortical subcortical language circuits. In particular, our data support view that damage to may result in from production deficits an intact seems crucial for supporting residual comprehension ability patients post-stroke aphasia. Collectively, these imply importance during disorder subacute stage stroke.

Background The prediction of post-stroke language function is essential for the development individualized treatment plans based on personal recovery potential aphasic stroke patients. Objective To establish a framework integrating information connectivity disruption network routinely collected clinical magnetic resonance (MR) images into Random Forest modeling to predict function. Methods Language was assessed in 76 patients from Non-Invasive Repeated Therapeutic Stimulation Aphasia Recovery trial, using Token Test (TT), Boston Naming (BNT), and Semantic Verbal Fluency (sVF) as primary outcome measures. Individual infarct masks were superimposed onto diffusion tensor imaging tractogram reference set calculate Change Connectivity scores language-relevant gray matter regions estimates structural disruption. Multivariable models derived Results explained moderate high amount variance at baseline follow-up TT (62.7% 76.2%), BNT (47.0% 84.3%), sVF (52.2% 61.1%). Initial non-verbal cognitive ability most important variables additional variance, resulting error increase up 12.8% with variable omission. Left middle temporal gyrus (12.8%) supramarginal (9.8%) identified among nodes. Conclusion adds predictive value beyond lesion volume, initial function, ability. Obtaining routine MR constitutes significant advancement toward practical application.
