The relationship between gut microbiota composition and allergen exposure in children with allergic rhinitis (AR) remains insufficiently explored, particularly concerning variations at different allergen-specific IgE (sIgE) levels the distinction monosensitization polysensitization to allergens. We employed metagenomic shotgun sequencing compare fecal of 50 healthy controls (HC) 88 AR induced by house dust mites (HDM-AR). further examined differences among HDM-AR subgroups extremely high mite-sIgE (EH-HDM), HDM-sIgE (H-HDM), as well monosensitized (mono-HDM) polysensitized (poly-HDM) individuals. While no significant overall microbiome diversity were observed HC groups, a notable increase relative abundance Streptococcus sanguinis within genus was identified AR. Further analysis revealed enrichment EH-HDM group, highlighting an increased salivarius. Functional gene via KEGG pathways indicated substantial salivary secretion pathway. Additionally, comparisons HC, mono-HDM, poly-HDM salivarius emerged key differential species, showing marked mono-HDM group. Our study suggests that specific bacterial strains, salivarius, may be potential biomarkers for assessing varying degrees patterns HDM sensitization. House dust mites (HDMs) are the most common cause of atopic sensitivities and allergic diseases worldwide. Therefore, we decided to conduct a meta-analysis randomized clinical trials evaluate effect different methods HDM avoidance. PubMed, Web Science, Cochrane Central Register Controlled Trials databases were searched find articles control-compared trials, which investigated following analyzed outcomes: total amount (g); Der1 (Der p1 plus Der f1) concentrations (µg/g); p2 f1 measurements peak expiratory flow (PEF) (L/min); respiratory, pulmonary, or nasal symptoms according various scales, including visual analog scale; exacerbations; Asthma Control Questionnaire score measurements; change in quality life scales (overall change, activity symptom emotional function change). Evidence of house dust mite immunotherapy in paediatric asthma: a review DOI Creative Commons

Asthma is a prevalent chronic respiratory condition in children, often exacerbated by allergic reactions, with house dust mites (HDMs) being significant trigger. Traditional asthma management primarily involves inhaled corticosteroids and bronchodilators, which do not address the underlying mechanisms. Allergen immunotherapy, including subcutaneous sublingual immunotherapy (SLIT), has emerged as strategy to induce immune tolerance allergens. This review evaluates efficacy of HDM paediatric asthma, focusing on reductions exacerbations, improvements lung function, decreases medication use, enhancements quality life (QoL). The highlights that both (SCIT) SLIT significantly reduce exacerbations SCIT showing superior function improvement. Combination therapies, particularly biologics, demonstrate enhanced outcomes, use reductions. also been shown improve more effectively than SLIT, children high baseline levels HDM-specific IgE. In terms safety, are generally well-tolerated, though associated localized systemic can be mitigated combination biologics. Furthermore, enhances QoL patients reducing symptoms improving sleep, leading long-lasting benefits. Despite these positive there remain gaps knowledge, regarding optimal duration therapy long-term effects post-treatment. Future research should standardize treatment protocols, explore personalized approaches, investigate sustainability benefits fully optimize asthma.
