Fatigue is one of the most prevalent and debilitating non-motor symptoms Parkinson's disease (PD), affecting up to two-thirds patients significantly impacting quality life. This review provides a comprehensive analysis its complex pathophysiology, prevalence, clinical presentation, assessment methods, current management strategies. in PD linked dysfunction dopaminergic non-dopaminergic pathways, neuroinflammation, genetic predispositions, metabolic dysregulation. Reported prevalence rates range from 36% 60%, highlighting need for standardized tools universally accepted definition. Clinically, fatigue characterized by an overwhelming abnormal sense exhaustion, often preceding motor years. Distinguishing it sleepiness, apathy, depression crucial accurate diagnosis treatment. Assessment primarily relies on patient-reported scales, such as Severity Scale Disease Scale. Management includes both pharmacological non-pharmacological approaches, with medications like methylphenidate rasagiline showing potential, alongside physical exercise, cognitive behavioral therapy, sleep disorder management. Further research essential unravel underlying mechanisms, identify objective biomarkers, develop more effective, targeted treatments this burdensome symptom.
