Job satisfaction and willingness to stay are critical for workforce stability in a challenging healthcare environment. This study examined how relational coordination, key factor teamwork communication, influences outcomes among professionals bilingual, culturally mixed region of Italy. cross-sectional survey included general practitioners, hospital physicians, nurses, administrators from the South Tyrol Health Service, using ‘Relational Coordination Survey’ additional measures job stay. Continuity of care is fundamental to the efficacy primary healthcare, fostering trust, enhancing patient satisfaction, and improving health outcomes. However, implementation Ministerial Decree 77/2022, which advocates for team-based in multidisciplinary Community Health Centers, presents challenges these established principles. This article proposes strategies maintain continuity trust whilst supporting reform objectives, specifically tailored unique linguistic cultural context Autonomous Province Bolzano—South Tyrol. A synthesis regional healthcare reports, academic literature, practical insights from implementing 77/2022 was performed develop addressing such as ensuring continuity, minimizing administrative burdens, promoting general practitioner engagement. Strategies include establishing Centers integration hubs, assigning providers within teams, formalizing personalized contracts, leveraging digital tools collaboration, expanding roles nurses coordinators. Homogeneous waiting group (HWG) criteria are central to the patient referral process, guiding primary care physicians and hospitalists in directing specialists. This cross-sectional observational study, conducted South Tyrol, Italy, 2023, aimed assess implementation impact of HWG on healthcare from perspective general practitioners hospital physicians. Background: Ministerial Decree (D.M.) 77/2022 aims to reform Italy’s primary care system by establishing community health centres and integrating digital tools address healthcare access disparities workforce shortages. This review focuses on frailty assessment tools, innovations, challenges in the Autonomous Province of Bolzano, South Tyrol, emphasising interprofessional trust collaboration issues. Methods: Using a narrative custom approach guided SANRA checklist, this study synthesised findings from PubMed, official websites, regional surveys frailty, dynamics, collaboration, infrastructure Tyrol. Results: General practitioners (GPs) exhibited high professional motivation but expressed concerns about autonomy administrative burdens collaborative models. Trust issues between GPs specialists hinder cohesion coordination, highlighting need for structured inter-professional communication. Frailty assessments, such as PRISMA-7 tool, identify over 33% community-dwelling individuals aged 75 years older frail, necessitating targeted interventions. Digital adoption, particularly electronic records telemedicine, is slow because shortages limitations. Conclusions: The successful implementation D.M. Tyrol requires addressing challenges, improving trust, expanding infrastructure, into strategies. These measures are critical achieving more resilient, equitable, effective system.
