A epilepsia do lobo temporal (ELT) é uma síndrome neurológica comum, caracterizada por convulsões refratárias, frequentemente associada à esclerose hipocampal (EH). Compreender os mecanismos inflamatórios na ELT fundamental para desenvolver tratamentos mais eficazes, especialmente casos que não respondem às terapias convencionais. Com o objetivo de investigar a relação entre neuroinflamação e com EH, realizou-se revisão sistemática seguindo diretrizes PRISMA, amostra final 10 artigos 2019 2024 disponíveis Biblioteca Virtual em Saúde (BVS). Os resultados mostraram neuroinflamação, mediada citocinas como IL-1β, IL-6 proteína HMGB1, exacerba as crises epilépticas. ativação dos inflamassomas NLRP1 NLRP3 resposta imune microglia astrócitos estão ligadas degeneração neuronal no hipocampo. afeta integridade tecido hipocampal, neurogênese plasticidade sináptica, levando déficits cognitivos. Assim, modulação das vias inflamatórias surge possível estratégia terapêutica reduzir preservar função cognitiva, mas estudos são necessários otimizar manejo clínico.

Drug-resistant epilepsy (DRE) presents significant challenges in treatment and management. While seizure-related alterations peripheral immune players are increasingly recognized, the involvement of complement system remains insufficiently explored DRE. Epilepsy is a multifaceted neurological disorder characterized by recurrent seizures and associated with molecular immune alterations in key brain regions. The GASH/Sal (Genetic Audiogenic Seizure Hamster, Salamanca), genetic model for audiogenic epilepsy, provides powerful tool to study seizure mechanisms resistance predisposed individuals. This investigates the proteomic responses triggered kindling inferior colliculus, comparing non-responder animals exhibiting reduced severity following repeated stimulation versus naïve hamsters. To assess auditory pathway functionality, Auditory Brainstem Responses (ABRs) were recorded, revealing neuronal activity nerve of non-responders, while central processing remained unaffected. Cytokine profiling demonstrated increased levels proinflammatory markers, including IL-1 alpha (Interleukin-1 alpha), IL-10 (Interleukin-10), TGF-beta (Transforming Growth Factor beta), alongside decreased IGF-1 (Insulin-like 1) levels, highlighting systemic inflammation its interplay neuroprotection. Building on these findings, analysis identified 159 differentially expressed proteins (DEPs). Additionally, bioinformatic approaches, Gene Set Enrichment Analysis (GSEA) Weighted Co-expression Network (WGCNA), revealed disrupted pathways related metabolic inflammatory epileptic processes module potentially linked rise threshold seizures, respectively. Objectives This study aims to evaluate cognitive impairment utilizing the Montreal Cognitive Assessment (MoCA) scale, while also exploring correlation between and various serum biomarkers, including Brain-derived neurotrophic factor (BDNF), Beta Secretase-1 (BACE1), Vascular Endothelial Growth Factors (VEGF), Glial fibrillary acidic protein (GFAP), Interleukin-1 (IL-1β) in adults living with epilepsy. Methods In this study, 74 participants aged 18 50 years, who were visiting neurology outpatient consultations, included. The assessment was executed using MoCA test. Serum levels of BDNF, BACE1, VEGF, GFAP, IL-1β evaluated through ELISA patients without impairments. To determine association scores both Spearman Pearson analyses, as well linear regression, conducted. Results Among PWE, 61 exhibited determined by assessment. Noteworthy alterations detected across subscales, encompassing visuospatial executive functions, attention, language, abstraction, delayed recall, statistical significance established ( p < 0.05).
