Abstract Hypertension is a major global health concern associated with significant morbidity and mortality due to its effects on target organs such as the heart, kidneys, brain. Non-dipper hypertension, characterized by insufficient nocturnal blood pressure reduction leading sustained hemodynamic load, linked higher risks of end-organ damage. This study evaluates impact hypertension LA anatomy physiomechanics explores association strain parameters hypertensive heart disease nephropathy. prospective, single-center cohort included 135 participants: 35 healthy controls, 50 dipper non-dipper patients. All participants underwent 24-h ABPM, biochemical analysis, transthoracic echocardiography, including speckle-tracking novel technique for analysis. diameter,LASr, LAVI maximum were identified was significantly values reduced dipping compared hypertension. parameters(LASr, LAScd, LASct) lower in group. These reductions indicate impaired left atrial mechanics early cardiac dysfunction. Logistic regression revealed that diameter, LASr, variables disease, whereas only LASr analysis provides valuable insights into pathophysiology complications. The findings support use cost-effective, noninvasive biomarkers assessing associations nephropathy, enabling risk stratification. emphasizes importance enhanced monitoring tailored interventions high-risk
