Background Fiber is a potential therapeutic to suppress microbiota-generated uremic molecules. This study aimed determine if fiber supplementation decreased serum levels of molecules through the modulation gut microbiota in adults undergoing hemodialysis. Methods A randomized, double-blinded, controlled crossover was conducted. Following 1-week baseline, participants consumed muffins with added pea hull (PHF) (15 g/d) and control daily, each for 4 weeks, separated by 4-week washout. Blood stool samples were collected per period. Serum p -cresyl sulfate (PCS), indoxyl (IS), phenylacetylglutamine (PAG), trimethylamine N -oxide (TMAO) quantified LC–MS/MS, fecal profiled 16S rRNA gene amplicon sequencing specific taxa interest qPCR. Diabetic Kidney Disease (DKD) is a common and serious complication of diabetes, particularly Type 2 Diabetes Mellitus (T2DM), which significantly contributes to patient morbidity mortality. The limitations traditional treatments like ACE inhibitors ARBs in managing DKD progression highlight the need for innovative therapeutic strategies. This review examines impact various dietary patterns, such as Mediterranean diet, ketogenic intermittent fasting, DASH vegetarian on management DKD. Evidence suggests these diets can halt DKD, although further research needed confirm their long-term effectiveness safety. Personalized approaches tailored individual needs may enhance outcomes patients.
