Aim: The purpose of this retrospective observational study was to compare peripheral venous cannulation performed under ultrasound guidance by expe-rienced and inexperienced nurses. Material Methods: retrospective. Analysis done eighty-seven charts successful ultrasound-guided cannulations experienced operators over a 5-month period. Results: In the group, an average 1.31 puncture attempts were (median=1), in group that number 1.42 (median=1). One attempt enough for 76.6% (n=36) 72.5% (n=29) cannulations, respectively. Experienced nurses needed statistically signif i cantly less time successfully vessel (p=0.003). Conclusions: Implementing procedure inserting midline catheters should be based on properly conducted training supervised clinical experience. Performing adequate during reduced required decreased failures. Completing 20 50 course makes it possible achieve high success rate vascular access insertion with minimum attempts.

The aim of this study was to assess whether procalcitonin levels is a diagnostic tool capable accurately identifying sepsis and ventilator-associated pneumonia (VAP) even in critically ill COVID-19 patients. In retrospective, observational study, all patients who survived for ≥2 days single university hospital had at least one serum (PCT) value associated blood culture and/or from lower respiratory tract specimen available were eligible the study.Over research period, 184 recruited; 67 VAP/BSI occurred, with an incidence rate 21.82 episodes (95% CI: 17.18-27.73) per 1000 patient-days among included. At time positive microbiological culture, average PCT level 1.25-3.2 ng/mL found. Moreover, also subjects without cultures, altered 21.7% determinations, 1.04-5.5 ng/mL. Both PCT-72 h not linked diagnosis patients, according multivariable GEE models (aOR 1.13, 95% CI 0.51-2.52 PCT; aOR 1.32, 0.66-2.64 h).Elevated might always indicate bacterial superinfections or coinfections severe setting.

Secondary pneumonia occurs in 8-24% of patients with Coronavirus 2019 (COVID-19) infection and is associated increased morbidity mortality. Diagnosis secondary can be challenging. The purpose this study was to evaluate the use plasma microbial cell free DNA sequencing (mcfNGS) evaluation after COVID-19. We performed a single-center case series COVID-19 who underwent mcfNGS reported organisms identified, concordance available tests, clinical utility, outcomes. In 8/13 (61%) cases, detected 1-6 organisms, clinically significant identified 4 including Pneumocystis jirovecii, Legionella spp. Management changed 85% (11/13) based on results, initiation targeted therapy, de-escalation empiric antimicrobials, avoiding contingent escalation antifungals. may helpful identify pathogens causing pneumonia, opportunistic immunocompromised However, providers need carefully interpret test within context.
