Bergamo was the most severely affected Italian province at onset of 2020 COVID-19 pandemic. The liver transplant (LT) patient population should be among more sensitized to concept health prevention. Long-term data on both perception and outcomes SARS-CoV-2 vaccination in LT recipients since vaccine became available Italy are still lacking. From May October 2023, a survey actively followed our institution carried by local patient' advocacy (Associazione Amici del Trapianto di Fegato) define rate vaccinated subjects, infections self-reported COVID-19-related outcomes. Out consecutive 753 adult invited survey, 356 responded (47.3%) [71% male, 63 years old (20-85), performed mean 9 (1-26) before vaccination] were included analysis. All patients received first dose between December January 2022 (81.7% Cominarty®, 17.7% Spikevax®, 0.3% Vaxzevria® Jcovden®). In following years, adherence policy decreased progressively over time: second, third, fourth, fifth doses administered 99%, 94%, 72%, 22% October, 2023. total, 43 (12%) 93 (26%) reported episode after [13 (7-21) months] vaccination, respectively; none second infection cycles. Forty-six (13%) short-term post-vaccination mild adverse events but developed either acute or chronic rejection episodes hospitalization for symptoms. A total 64% resulted positive anti-nucleocapsid serological test vaccines safe effective recipients, underlining once again importance this special higher risk complications from communicable infectious diseases.
