Introduction: Under-immunization remains a major global public health concern. The World Health Organization (WHO) reports that approximately 20 million children globally are not fully vaccinated, with more than half of these residing in Africa. African countries including Cameroon face unique challenges achieving high vaccination coverage. This study aimed to determine the prevalence and determinants under-immunization among aged 0–59 months Buea, contribute effective national immunization policy. The COVID-19 pandemic disrupted healthcare delivery across the United States (U.S.), including childhood vaccine administration. This study analyzed data from National Health Interview Survey (NHIS), a nationally representative survey of U.S. population, assessing trends and predictors influenza vaccination uptake among children ≤ 5 years before amid pandemic. Influenza coverage declined significantly, 56% in 2019 to 46% 2022 (p < 0.001). Age-specific declines were notable, with rates dropping one-year-olds 68% 53%, two-year-olds 63% 49%, infants 31% 24% Logistic regression revealed African American had lower odds compared non-Hispanic White (OR = 0.70, p 0.001), while Asian higher 1.32, 0.018).
