ABSTRACT Observational studies have shown that the gut microbiota (GM) is associated with bone diseases, particularly calcium‐phosphorus metabolic demonstrating existence of a gut–bone axis. However, whether these associations are causal effects remains to be determined. This study employed bidirectional two‐sample Mendelian randomisation (MR) using summary data from Genome‐Wide Association Studies (GWAS) 211 microbial taxa and six diseases (osteoporosis, Osteopenia, osteonecrosis, osteomyelitis, hypoparathyroidism hyperparathyroidism) explore relationships their directionality. Comprehensive sensitivity analyses were conducted ensure robustness results, false discovery rate‐corrected p FDR < 0.05 was used as threshold support strong associations. Additionally, co‐localisation analysis consolidate findings. We identified 35 between GM 17 exhibiting positive 18 negative correlations. Furthermore, reverse MR indicated osteomyelitis elevated abundance two GMs ( 0.05, PP.H4 75%). No evidence horizontal pleiotropy or heterogeneity observed, further strengthened for relationships. The underscores critical role in influencing health through axis, paving way future therapeutic interventions targeting axis offering new directions research metabolism diseases.
