Genomic surveillance activity is a useful tool in epidemiologic investigations and monitoring of virus evolution. This study aimed to describe the COVID-19 outbreaks through SARS-CoV-2 genomic by whole genome sequencing. A cross-sectional was conducted using archived clinical samples confirmed laboratory-positive from June 2021 2022 tertiary center Malaysia. The were subjected phylogenetic tree constructed maximum likelihood method MEGA 11 software. data obtained paper, electronic, hospital information systems. total 86 successfully sequenced. revealed seven clusters, consisting 24 cases. Three clusters associated with health care workers care-associated individuals. Delta variants observed first three subsequently replaced Omicron variants. Whole sequencing robust reliable, enhancing investigations, leading identification preventing spreading among workers. Monitoring necessary viral dynamics maintain effectiveness public interventions.
