Background Rheumatoid arthritis (RA) is a chronic autoimmune disorder characterized by joint inflammation, pain, and progressive disability. Identifying biomarkers that accurately reflect disease severity crucial for effective management. Interleukin-6 (IL-6) pro-inflammatory cytokine involved in the pathogenesis of RA, systemic immune-inflammation index (SII) emerging as useful marker inflammation. This study aims to explore relationship between serum IL-6 levels, SII, various better predict RA patients. Objective To determine levels along with biomarkers, across different predicting Methods cross-sectional, observational was conducted at Mardan Medical Complex from January 2024 August 2024, involving 67 Clinical assessments included demographic data, activity (DAS28), pain (VAS), damage (Larsen score), functional status (HAQ-DI). Serum other such C-reactive protein (CRP), erythrocyte sedimentation rate (ESR), were measured through fasting blood samples. Statistical analyses, including density plots, scatter boxplots ANOVA, random forest models, performed associations all variables. Significance set p < 0.05. Results The patients (mean age: 41.79 ± 10.51 years, 53.73% male). Elevated (mean: 80.28 35.27 pg/mL) strongly associated severity. Patients DAS28 > 5.5 had over 100 pg/mL, while those remission around 40 pg/mL. correlated (100 pg/mL severe cases) (over 120 pain). metabolic cardiovascular comorbidities highest particularly diabetes hypertension (98.6 or (119.3 pg/mL). CRP (r = 0.65), ESR 0.51), SII 0.62). Regression confirmed an independent predictor (p 0.001), being key predictors. Conclusion serve critical markers RA. Addressing these may lead more targeted therapeutic strategies managing progression.
