Autoimmune hepatitis is a chronic liver disease marked by immune-mediated inflammation, necrosis, and the potential to progress cirrhosis if not treated. This case report presents rare atypical presentation of autoimmune in Nigerian adolescent girl, highlighting diagnostic challenges resource-limited settings. The unique owing absence jaundice, common symptom dysfunction, features such as delayed menarche bilateral leg swelling. Case patient was 16-year-old Black female Igbo ethnicity from Nigeria, who presented with 6-month history swelling menarche. She had no jaundice abdominal pain, she significant past medical history. initially misdiagnosed, delaying appropriate management. Following comprehensive workup, including function tests, imaging, autoantibody testing, which were positive for antinuclear antismooth muscle antibodies, correctly diagnosed type 1 hepatitis. Treatment initiated corticosteroids (prednisolone) azathioprine, resulted clinical improvement. However, her serum albumin levels remained low result preexisting cirrhosis. highlights adolescents, particularly regions where infectious diseases are more commonly suspected. It emphasizes need increased awareness better resources improve early detection management sub-Saharan Africa. Early intervention immunosuppressive therapy essential, even classic liver-related symptoms, prevent progression advanced disease.

Objective Histone deacetylase 6 (HDAC6) has been confirmed to participate in the regulation of liver fibrosis (LF) progression. This study aims explore role and mechanism HDAC6 LF process. Material Methods Serum samples were collected from cirrhosis (LC) patients normal healthy individuals. Human hepatic stellate cells (HSC; LX-2) stimulated with transforming growth factor β1 (TGF-β1) mimic cell models. The levels HDAC6, ribosomal protein S5 (RPS5), embryonic lethal abnormal vision like 1 (ELAVL1), fibrosis-related markers determined by quantitative real-time polymerase chain reaction or western blot. Cell proliferation invasion detected using counting kit 8 assay, 5-ethynyl-2’-deoxyuridine Transwell assay. contents inflammatory factors examined enzyme-linked immunosorbent Furthermore, co-immunoprecipitation RNA immunoprecipitation assays performed assess interaction between RPS5 ELAVL1. effect ELAVL1 knockdown on mRNA stability was evaluated Actinomycin D treatment Results showed increased expression LC patients. reduced TGF-β1-induced LX-2 proliferation, invasion, fibrosis, inflammation. Moreover, acetylation RPS5, reversed inhibition si-HDAC6 Meanwhile, interacted stabilize its mRNA, thus inhibiting expression. Conclusion Our data revealed that ELAVL1-stabilized promoted HSC activation repressing acetylation, providing a novel target for alleviating

Background/Objectives: Given the rising incidence of early-onset endometrial cancer diagnosed before age 50 years, this study examined whether non-alcoholic fatty liver disease (NAFLD) served as an independent risk factor for cancer, irrespective obesity status. Methods: This nationwide cohort included 2,311,949 Korean women aged 20–39 years who underwent health screenings from 2009 to 2012. NAFLD severity was classified based on index: none (<30), moderate (30–59), and severe (≥60). Multivariable-adjusted Cox proportional hazards models were applied estimate adjusted hazard ratios (aHRs) cancer. Results: During a median follow-up 7.6 1289 developed The cumulative significantly higher in with than those without (log-rank p < 0.001). A dose-dependent association observed, increased corresponding greater (aHR [95% confidence interval (CI)]: NAFLD, 2.38 [1.99–2.85]; 5.39 [4.44–6.53]; trend 0.01). Compared non-obese aHRs 2.53 [2.11–3.05] 1.66 [1.10–2.52] obese 4.30 [3.60–5.13] (synergy index = 1.50, Conclusions: independently associated both women. Furthermore, young exhibited synergistically elevated risk. Early identification management may help mitigate rapidly growing burden

Abstract Autoimmune hepatitis (AIH) is a chronic liver disease characterized by immune-mediated inflammation, necrosis, and potential progression to cirrhosis if not managed appropriately. This case report describes rare presentation of AIH in 16-year-old Nigerian adolescent with atypical features, including delayed menarche bilateral leg swelling, but without jaundice which common symptom dysfunction. The patient was initially misdiagnosed other conditions before being correctly identified as having Type 1 Hepatitis following appropriate diagnostic evaluation, included function tests, imaging, autoantibody tests. She responded favourably treatment corticosteroids azathioprine her serum albumin remained low due pre-existing cirrhosis. highlights the challenges AIH, particularly resource-limited settings where diagnosis infectious diseases often overshadows autoimmune conditions. delay emphasizes need for increased awareness improved resources sub-African region. Early intervention immunosuppressive therapy crucial managing preventing advanced disease. further importance high index suspicion adolescents unexplained disease, even absence classic symptoms.
