Hypoxia Imaging in Lung Cancer: A PET-Based Narrative Review for Clinicians and Researchers

Background:
Hypoxia
plays
a
critical
role
in
lung
cancer
progression
and
treatment
resistance
by
contributing
to
aggressive
tumor
behavior
poor
therapeutic
response.
Molecular
imaging,
particularly
positron
emission
tomography
(PET),
has
become
an
essential
tool
for
noninvasive
hypoxia
detection,
providing
valuable
insights
into
biology
aiding
personalized
strategies.
Objective:
This
narrative
review
explores
recent
advancements
PET
imaging
detecting
cancer,
with
focus
on
the
development,
characteristics,
clinical
applications
of
various
radiotracers.
Findings:
Numerous
PET-based
radiotracers
have
been
investigated,
each
distinct
pharmacokinetics
capabilities.
Established
tracers
such
as
18F-Fluoromisonidazole
(18F-FMISO)
remain
widely
used,
while
newer
alternatives
like
18F-Fluoroazomycin
Arabinoside
(18F-FAZA)
18F-Flortanidazole
(18F-HX4)
demonstrate
improved
clearance
image
contrast.
Additionally,
64Cu-ATSM
gained
attention
its
rapid
uptake
selectivity.
The
integration
hybrid
modalities,
PET/CT
PET/MRI,
enhances
spatial
resolution
functional
interpretation,
making
promising
approach
guiding
radiotherapy,
chemotherapy,
targeted
therapies.
Conclusions:
offers
significant
potential
diagnosis,
planning,
response
assessment.
However,
challenges
remain,
including
tracer
specificity,
quantification
variability,
standardization
protocols.
Future
research
should
developing
next-generation
enhanced
optimizing
methodologies,
leveraging
multimodal
approaches

utility

Background:
Although
white
blood
cell-related
indices
are
established
prognostic
markers
in
lung
cancer,
the
significance
of
red
cell
(RBC)
indices—mean
corpuscular
volume
(MCV),
mean
hemoglobin
(MCH),
and
concentration
(MCHC)—remains
unclear.
This
study
assessed
value
RBC
for
predicting
survival
outcomes
patients
who
underwent
curative-intent
surgery
stage
I–IIIA
non-small
cancer
(NSCLC).
Methods:
retrospective
analysis
437
evaluated
MCV,
MCH,
MCHC,
modified
Shine
Lal
Index
(mSLI),
calculated
as
(MCV2
×
MCH)
0.0001,
using
Cox
regression
analysis.
Model
performance
was
various
metrics,
including
concordance
index
(C-index)
integrated
discrimination
improvement
(IDI).
Results:
In
multivariate
analysis,
each
tested
separately
an
overall
(OS)
predictor
models
that
consistently
included
age,
American
Society
Anesthesiologists
Physical
Status
(ASA-PS),
pleural
invasion,
tumor–node–metastasis
(TNM)
stage,
Noble
Underwood
(NUn)
score.
Given
its
superior
predictive
performance,
mSLI
model,
which
incorporates
addition

other
covariates,

outperformed
baseline
TNM
staging
model
