Ferroptosis is a form of regulated cell death that damages neurons in the central nervous system. In this study, we aimed to construct ferroptosis-related gene signatures temporal lobe epilepsy with hippocampal sclerosis (TLE-HS) and explore their diagnostic role TLE-HS. The GSE205661 dataset was acquired for training purposes, while GSE71058 obtained serve as validation dataset. Subsequently, differentially expressed genes (FR-DEGs) TLE-HS were further analyzed. We used weighed co-expression network analysis (WGCNA) algorithm, single-factor logistic regression analysis, LASSO algorithm screen characteristic FR-DEGs. Then, receiver operating (ROC) evaluate value these disease diagnosis. Finally, long non-coding RNA (lncRNA)-microRNA (miRNA)-messenger (mRNA) constructed. identified 141 FR-DEGs TLE-HS, enriched T-cell activation involved immune response signaling pathways related lipids atherosclerosis. Further WGCNA performed select 47 overlapping FR-DEGs, which significantly 13 biological processes 14 Kyoto Encyclopedia Genes Genomes (KEGG) pathways, including negative regulation apoptotic process ferroptosis. Four genes, namely PDK4, SMPD1, GPT2, METTL14, signature Moreover, ROC derived from four predicting had an area under curve (AUC) 0.988 0.929, respectively. Furthermore, lncRNA-miRNA-mRNA constructed 4 consisted 5 lncRNAs miRNAs. based on found be strong predictor they may represent valuable therapeutic targets
