In Zambia, the true extent of SARS-CoV-2 infections is unknown because initial surveillance focused on patients with symptoms or severe disease. Antenatal sentinel had not been used to assess infection trends. The ANC COVID-19 study sought determine seroprevalence and vaccine uptake among pregnant women. We provide insight into implementation, challenges encountered, best practices, lessons learned. A repeated cross-sectional survey was implemented at 39 health facilities in four districts from September 2021 2022. Pregnant women aged 15–49 years were enrolled their first antenatal care visits. An electronic questionnaire gathered demographics other related information consenting participants. dried blood sample collected detect IgG antibodies using a multiplex bead assay. Seropositive results categorized as infection, vaccination based anti-RBD anti-nucleocapsid test results. Problems root causes identified they occurred. Practical problem-solving strategies devised, implemented, monitored ensure that goals accomplished. primary analysis, 7% 9,221 samples participants tested missing. 9,111 assessed. Approximately 64% cumulatively seropositive for antibodies. Seroprevalence increased 27.8% 56.6% July observed an increase coverage (0.5–27%) over time. Delta variant reallocation between two partners delayed activities cost implementation (e.g., purchase additional calibration validation kits DBS cards). Protocol deviations attributed lack experience conducting research but, district RAs repeatedly trained facility staff enhance knowledge. Incorporating routine feasible potentially sustainable when existing system infrastructure, human resources, systems are leveraged. Yet, careful planning needed anticipate high-quality data collection.
