The triglyceride-glucose (TyG) index and its related indexes (TyG-BMI, TyG-WC, TyG-WHtR) are effective markers for screening metabolic diseases like insulin resistance (IR). However, few studies have explored the relationship between TyG with bone density (BMD), osteopenia, osteoporosis. This is a cross-sectional study that involved 1,303 adults aged 50 years above from National Health Nutrition Examination Survey 2007–2010, 2013–2014. In multivariable-adjusted model, linear regression analysis logistic demonstrated significant positive correlation BMD negative osteopenia/osteoporosis in femoral neck, lumbar spine, total hip region. Trend further confirms these associations (p < 0.05). Restricted cubic spline showed nonlinear osteopenia/osteoporosis. Sensitivity analyses confirmed robustness of associations. reveals complex osteoporosis, indicating potential link IR health. offer new perspective diagnosis, prevention, treatment
