Low-density lipoprotein cholesterol (LDL-C) is a well-established biomarker in the management of dyslipidemia. Therefore, we aimed to evaluate concordance LDL-C-estimating equations with direct enzymatic measurement diabetic and prediabetic populations. The data 31,031 subjects included study were divided into prediabetic, diabetic, control groups according HbA1c values. LDL-C was measured by homogenous assay calculated Martin-Hopkins, Martin-Hopkins extended, Friedewald, Sampson equations. statistics between measurements estimations obtained evaluated. Recent studies have shown that Friedewald underestimates low-density lipoprotein cholesterol (LDL-C) at lower levels, which could result in undertreatment of high-risk patients. A novel method (Martin/Hopkins) using a patient-specific conversion factor provides more accurate LDL-C levels. Importance

Low-density lipoprotein cholesterol (LDL-C) is typically estimated with the Friedewald or Martin/Hopkins equation; however, if triglyceride levels are 400 mg/dL greater, laboratories reflexively perform direct LDL-C (dLDL-C) measurement. The use of chemical assays and estimation via National Institutes Health Sampson equation not well validated, data on accuracy at higher limited.

Objective

This cross-sectional study evaluated consecutive patients clinical sites across US patient lipid distributions representative population in Very Large Database Lipids from January 1, 2006, December 31, 2015, mg/dL. Data analysis was performed November 9, 2020, March 23, 2021.

Background: Alternatives to the Friedewald low-density lipoprotein cholesterol (LDL-C) equation have been proposed. Objective: To compare accuracy of available LDL-C equations with ultracentrifugation measurement. Methods: We used second harvest Very Large Database Lipids (VLDbL), which is a population-representative convenience sample adult and pediatric patients (N = 5,051,467) clinical lipid measurements obtained via vertical auto profile (VAP) method between October 1, 2015 June 30, 2019. performed systematic literature review identify compared their according guideline-based classification. also by median error versus ultracentrifugation. evaluated overall stratified age, sex, fasting status, triglyceride levels, as well in atherosclerotic cardiovascular disease, hypertension, diabetes, kidney inflammation, thyroid dysfunction. Results: Analyzing 23 identified 5,051,467 (mean±SD 56±16 years; 53.3% women), Martin/Hopkins most accurately classified correct category (89.6%), followed Sampson (86.3%), Chen (84.4%), Puavilai (84.1%), Delong (83.3%), (83.2%) equations. Background Low-density lipoprotein cholesterol (LDL-C) is an important biomarker for determining cardiovascular risk and regulating lipid lowering therapy. Therefore, the accurate estimation of LDL-C concentration essential in disease diagnosis prognosis. Sampson recently proposed a new formula LDL-C. However, little known regarding validation this formula. Objectives This study aimed to validate with other well-known formulas Turkish population, composed adults. Methods A total 88,943 participants above 18 years old at Sivas Cumhuriyet University Hospital (Sivas, Turkey) were included study. was directly measured by homogeneous assays, i.e., Roche, Beckman Siemens estimated Friedewald’s, Martin-Hopkins’, extended Martin-Hopkins’ Sampson’s formulas. The concordances between estimations obtained direct measurements evaluated both general separately LDL-C, TG non-HDL-C sublevels. Linear regression analysis applied residual error plots generated each measurement method. Coefficient determination ( R 2 ) mean absolute deviations also calculated. Results results showed that Martin-Hopkins approach provided most concordant assays estimation. highest calculated median statistics from our own population.
