Abstract OBJECTIVE: The aim of this study was to systematically assess the predictive ability multiple weight-related indicators (including body mass index (BMI), waist circumference (WC), and triglyceride-glucose (TyG)) on cognitive decline, explore differences in sensitivity these different gender populations. By comparing effects each index, hopes identify more clinically valuable screening tools for early detection impairment, thus providing a scientific basis public health interventions. METHODS: Based China Health Retirement Longitudinal Study (CHARLS) database, 8,255 middle-aged elderly subjects aged 45 98 years old who met criteria were screened total 14 indices, including BMI, WC, weight-adjusted (WWI), waist-to-height ratio (WHtR), TyG its derivatives (TyG-BMI, TyG-WC, TyG-WHtR), visceral adiposity (VAI), size (ABSI), roundness (BRI), lipid accumulation (LAP), (LAP). circularity (CI), Chinese (CVAI). impairment assessed using subject's work characteristic curve (ROC) analysis, optimal cut-off value, sensitivity, specificity area under (AUC) calculated. In addition, associations between severe (SCI) analyzed binary logistic regression models adjusted confounders such as age, blood pressure, chronic diseases improve robustness models. RESULTS: Among 8255 included, AUC values most ranged from 0.500 0.590, with limited overall power. Sex-stratified analysis showed that BMI (AUC=0.591), TyG-BMI (AUC=0.585), WC (AUC=0.569) had high value male group, while WWI (AUC=0.588), (AUC=0.572) performed relatively better. based best cutoff after adjusting diseases, among men, (OR=1.31, 95% CI: 1.12-1.54), (OR=1.21, 1.03-1.41), (OR=1.36, 1.16- 1.60), TyG-WC (OR=1.23, 1.06-1.44), TyG-WHtR (OR=1.19, 1.02-1.39), VAI (OR=1.25, 1.07-1.46) LAP (OR=1.24, 1.06-1.44) all significantly positively correlated SCI (p < 0.05). female (OR = 1.49, 1.26-1.76), 1.39, 1.17-1.65), 1.43, 1.21-1.69), 1.40, 1.19-1.66) 1.36, 1.16-1.61) likewise significant (P results indicated compared traditional single weight indicators, TyG-BMI, CVAI could reflect distribution, fat metabolic characteristics comprehensively, have higher application identification risk. Objectives: The longitudinal relationship between sarcopenia, depression, and cognitive impairment has been insufficiently studied in China. This study aimed to characterize the association sarcopenia mediating role of depression using nationally representative data. Methods: 7091 middle-aged older adults were analyzed from China Health Retirement Longitudinal Study (CHARLS) across three waves (2011, 2013, 2015). Cognitive trajectories modeled a group-based trajectory model (GBTM), while multivariable ordinal logistic regression was employed evaluate associations with trajectories. depressive symptoms assessed through bootstrap mediation analysis cross-lagged panel modeling (CLPM). Results: Trajectory identified four distinct function patterns: “High Stable” (n = 2563, 36.73%), “Middle group 2860, 38.76%), Decline” 1280, 18.62%), “Low 388, 5.90%). Sarcopenia associated [Overall: OR (95%CI) 0.315 (0.259, 0.382) 0.417 (0.380, 0.459)]. Mediation indicated that accounted for 11.78% demonstrated significant pathway “T1 → T2 T3 sarcopenia”, 5.31% total effect. Conclusions: Our trajectories, significantly worse over time.
