Persons diagnosed with dementia are often faced challenges related to polypharmacy and inappropriate medication use could benefit from regular reviews. However, the of such reviews has not been examined in this population. Therefore, current scoping review was designed identify gaps knowledge regarding impact on clinical outcomes older adults dementia. To compare the implementation of collaborative dementia care management (cDCM) across two settings regarding patients' characteristics, unmet needs, and impact on health-related quality life (HRQoL) costs. This analysis was based data from DCM:IMPact study, implementing cDCM in a physician network (n = 22 practices) day centers (DCC) for community-dwelling persons with over 6 months Germany. Participants completed comprehensive assessments at baseline after, soliciting sociodemographic clinical HRQoL healthcare resource utilization. Patient characteristics needs were analyzed descriptively, costs assessed using multivariable regression models. Abstract Background and Objectives The University of California, San Francisco Memory Aging Center (UCSF-MAC) led the development tested a collaborative care model delivered by lay team navigators (CTNs) with support from multidisciplinary known as Care Ecosystem (CE).
