compare
the
implementation

collaborative
dementia
care
management
(cDCM)
across
two
settings

Abstract
Background
and
Objectives
The
University
of
California,
San
Francisco
Memory
Aging
Center
(UCSF-MAC)
led
the
development
tested
a
collaborative
care
model
delivered
by
lay
team
navigators
(CTNs)
with
support
from
multidisciplinary
known
as
Care
Ecosystem
(CE).
We
report
results
application
CE
within
non-academic
healthcare
system.
Research
Design
Methods
at
HealthPartners
consisted
two
CTNs,
social
worker,
an
RN,
program
coordinator,
behavioral
neurologist.
Intake
forms
were
developed
to
collect
demographic,
baseline,
annual
longitudinal
data
one
year
related
dementia
severity
partner
status.
Experience
surveys
completed
every
6
months
participating
partners.
All
was
entered
into
REDCap.
Results
A
total
570
PWD-caregiver
dyads
recruited
CE:
53%
PWDs
female,
average
age
75.2

rural
communities.

173
assessed
year,
58%
partners
responded
surveys,
67%
those
provided
about
their
experience
through
survey
data.
At
progressed
in
disease
functional
impairment,
although

mood
remained
unchanged.
