Abstract Objectives This study aimed to screen oral squamous cell carcinoma (OSCC) diagnostic and prognostic candidates investigate the potential functions mechanisms of in chemoresistance OSCC lines. Oral squamous cell carcinoma (OSCC) develops on the mucosal epithelium of oral cavity. It accounts for approximately 90% malignancies and impairs appearance, pronunciation, swallowing, flavor perception. In 2020, 377,713 OSCC cases were reported globally. According to Global Cancer Observatory (GCO), incidence will rise by 40% 2040, accompanied a growth in mortality. Persistent exposure various risk factors, including tobacco, alcohol, betel quid (BQ), human papillomavirus (HPV), lead development potentially malignant disorders (OPMDs), which are lesions with an increased developing into OSCC. Complex multifactorial, oncogenesis process involves genetic alteration, epigenetic modification, dysregulated tumor microenvironment. Although therapeutic interventions, such as chemotherapy, radiation, immunotherapy, nanomedicine, have been proposed prevent or treat OPMDs, understanding mechanism facilitate identification prognostic thereby improving efficacy treatment patients. Oral squamous cell carcinoma (OSCC), the eighth most prevalent cancer in world, arises from interaction of multiple factors including tobacco, alcohol consumption, and betel quid. Chemotherapeutic agents such as cisplatin, 5-fluorouracil, paclitaxel have now become first-line options for OSCC patients. Nevertheless, patients eventually acquire drug resistance, leading to poor prognosis. Cisplatin resistance poses a substantial hurdle in effectively treating head and neck squamous cell carcinoma (HNSCC). Utilizing multiple tumor models examining an internal HNSCC cohort, squalene epoxidase (SQLE) is pinpointed as key driver of chemoresistance tumorigenesis, operating through cholesterol-dependent pathway. Comprehensive transcriptomic analysis reveals that SQLE essential for maintaining c-Myc transcriptional activity by stabilizing the protein averting its ubiquitin-mediated degradation. Mechanistic investigation demonstrates inhibition diminishes Akt's binding affinity to lipid rafts via process, subsequently deactivating raft-localized Akt, reducing GSK-3β phosphorylation at S9, increasing T58, ultimately leading destabilization. Importantly, employing Sqle conditional knockout mouse model, SQLE's critical role initiation progression established. The preclinical findings demonstrate potent synergistic effects combining terbinafine cisplatin arresting growth. These discoveries not only provide novel insights into underlying mechanisms SQLE-mediated tumorigenesis but also propose promising therapeutic avenue patients unresponsive conventional cisplatin-based chemotherapy.
