Bipolar disorder is heterogeneous in phenomenology, illness trajectory, and response to treatment. Despite evidence for the efficacy of multimodal-ity interventions, majority persons affected by this do not achieve sustain full syndromal recovery. It eagerly anticipated that combining datasets across various information sources (e.g., hierarchical "multi-omic" measures, electronic health records), analyzed using advanced computational methods machine learning), will inform future diagnosis treatment selection. In interim, identifying clinically meaningful subgroups with having differential specific treatments at point-of-care an empirical priority. This paper endeavours synthesize salient domains clinical characterization adult patient bipolar disorder, overarching aim improve outcomes informing management considerations. Extant data indicate characterizing select provides actionable guides shared decision making. For example, it robustly established presence mixed features - especially during depressive episodes physical psychiatric comorbidities informs treatment, suicide risk. addition, early environmental exposures sexual abuse, emotional neglect) are highly associated more complicated presentations, inviting need developmentally-oriented integrated approaches. There have been significant advances validating subtypes I vs. II disorder), particularly regard pharmacological interventions. As other severe mental disorders, social functioning, interpersonal/family relationships internalized stigma relevant relapse risk, outcomes, quality life. The elevated standardized mortality ratio completed suicidal behaviour invites domain all patients. framework describe above domains, providing a synthesis extant literature recommendations support tools metrics can be implemented point-of-care.

The importance of diet and the gut-brain axis for brain health cognitive function is increasingly acknowledged. Dietary interventions are tested their potential to prevent and/or treat disorders. Intermittent fasting (IF), abstinence or strong limitation calories 12 48 h, alternated with periods regular food intake, has shown promising results on neurobiological in animal models. In this review article, we discuss benefits IF possible effects prevention progress brain-related disorders animals humans. We do so by summarizing which through metabolic, cellular, circadian mechanisms lead anatomical functional changes brain. Our shows that there no clear evidence a positive short-term effect cognition healthy subjects. Clinical studies show epilepsy, Alzheimer's disease, multiple sclerosis disease symptoms progress. Findings from Parkinson's ischemic stroke, autism spectrum disorder, mood anxiety could benefit IF. Future research should disentangle whether hold true regardless age presence obesity. Moreover, variations patterns, total caloric intake specific nutrients may be relevant components success. Longitudinal randomized clinical trials (RCTs) will provide window into long-term development diseases.

Symptoms of depression and anxiety frequently co-occur, but traditional discrete-time models fail to capture their causal interactions. To explore the dynamic relationship between these symptoms, we applied two advanced methodologies—non-Gaussian direction dependence analyses continuous-time structural equation modeling—across therapist-guided internet-based cognitive-behavioral therapy (iCBT) samples general-population cohorts ( N = 22,530). Our findings revealed that in iCBT, neither nor exhibited dominance; instead, changes were driven by shared transdiagnostic processes. In general population, showed unidirectional dominance over anxiety; stable symptom levels sustained time-invariant factors multiple years. Overall, this large-scale study suggests interplay is primarily processes alongside primacy depression. These insights underscore importance non-Gaussian modeling understanding mental-health comorbidities advocate for practices treating both anxiety.

Introduction: Schema therapy (ST) reduces depressive symptoms, but clinical trials have not investigated its effectiveness for patients suffering from severe forms of depression and high rates comorbidities. There is demand exploring improving treatments this patient group. The objective the current study was to evaluate whether ST more effective than individual supportive (IST) noninferior compared with cognitive behavioral (CBT) in treating depression. Methods: For trial, medicated were recruited inpatient day clinic settings. major inclusion criteria age between 18 75 years primary diagnosis without psychotic symptoms. A total 292 participants randomized ST, CBT, or IST received 7 weeks psychotherapy (up 14 group sessions). outcome change severity after treatment measured by Beck Depression Inventory-II. Primary test efficacy superiority over IST. Secondary noninferiority CBT. Multilevel modeling conducted. results at 6-month follow-up explored. Results: Across treatment, superior analyses completer showed similar results. However, clinically relevant Conclusion: as part a psychiatric care program being represents potentially useful addition therapeutic repertoire efficacy, including long-term should be evaluated further.
