Background: The treatment of articular cartilage damage has always represented a problem considerable practical interest for orthopedics. Over the years, many surgical techniques have been proposed to induce growth repairing tissue and limit degeneration. In 1994, turning point occurred: implanted autologous cells paved way new option based more on regeneration than repair. Objectives: This review aims outline biological clinical advances, from use mature adult chondrocytes cell-derived products, going through progenitor derived bone marrow or adipose their concentrates Moreover, it highlights relevance gene therapy as valuable tool successfully implementing current regenerative treatments, overcoming limitations local delivery factors. Background: Chondrocyte-based cell therapy remains a promising method for cartilage repair, despite limitations faced during the last 30 years. Purpose/Hypothesis: This work presents hyaline-like bioengineered beads from donor chondrocytes as novel treatment option lesions. It was hypothesized that implanted minigrafts would be able to treat lesions by complete fusion among themselves and integration with surrounding tissue. No tissue rejection expected because of cartilage’s reported immunological privilege. Study Design: Controlled laboratory study. Methods: Allogenic hyaline characteristics were produced frozen minipig donor. A total 8 Göttingen minipigs underwent implantation into 10 mm–diameter full-thickness chondral (3 lesions/knee). Animals sacrificed at 6 weeks (n = 2) months 6) after implantation. The safety efficacy assessed macroscopic histological analyses well magnetic resonance imaging. Results: signs acute or chronic observed in any study animals upon For months, imaging results showed better coverage grafted compared empty (control) When maintained lesion, subchondral bone native observed. Repair quality evidence zonal arrangement months’ follow-up. Additionally, 17) had repair scores than 7) (mean inverse Goebel score, 4.24 5.57, respectively). Graft-filled only slight superiority Bern 5.76 5.43, Conclusion: hold potential an advanced medicinal product treating 1-step surgery established efficacy. Clinical Relevance: successful preclinical highlights allogenic repair. Moreover, using may allow reduced patient morbidity surgery. Hence, this is suitable large older patients currently being evaluated phase I/IIa clinical trial.

Abstract Purpose Chondrocyte‐based cell therapies are effective for the treatment of chondral lesions, but remain poorly indicated diffuse lesions in context early osteoarthritis (OA). The aim this study was to develop a protocol obtain chondroprogenitor cells suitable within OA. Methods Cartilage were expanded at low density human platelet lysate (hPL). A test performed exclude senescence. expression surface cluster differentiation 146, 166, major histocompatibility complex (MHC)‐I and MHC‐II genes interest evaluated, as well trophic potential these cells, by assessment lubricin matrix production. immunomodulatory assessed through their co‐culture with macrophages. Results hPL showed higher proliferation rate than standard‐density no replicative senescence, immunogenicity lubricin. Moreover, they presented an increased chondrogenic antihypertrophic markers, superior deposition if compared cultured standard density. induced on macrophages upregulation CD206, although increase CD163 observed presence low‐density cells. Conclusions These findings lay grounds explore clinical usefulness cartilage treat OA joints both autologous allogenic use. Level Evidence Not applicable.

Abstract Purpose A cell‐free biomimetic osteochondral scaffold was developed to treat cartilage knee lesions, with positive clinical results documented in small case series. However, clear evidence on patient and lesion characteristics that might affect the outcome is still lacking. The aim of this study analyse a large cohort patients treated investigate factors could influence outcome. Methods Two hundred three (mean age 30.7 ± 10.9 years) were prospectively evaluated at baseline, 6‐, 12‐ 24‐month follow‐up. analysed using International Knee Documentation Committee (IKDC) score, activity level assessed Tegner score. outcomes analysed. Results Mild severe adverse reactions found 39.0% 1.5% patients, respectively. failure rate 2.0%, increasing 12.3% when including also failures. IKDC subjective score increased from 43.3 15.9 61.0 16.2 6 months, 68.3 18.5 12 months 73.8 18.3 24 ( p < 0.0005). improved 2.5 1.7 4.2 0.0005), without reaching pre‐injury (6.0 2.2) objective changed 68.5% normal nearly knees before treatment 90.1% months. At showed correlation ρ = −0.247; women had lower as well patellar lesions 0.002). Previous surgery correlated 0.003), while better osteochondritis dissecans (OCD) compared degenerative 0.001). Conclusion This safe effective for offering 2 years low rate. Better observed younger femoral condyles OCD, joints affected by who underwent previous surgery, may expect results. Mosaicplasty is a relatively challenging procedure used in the management of focal osteochondral lesions joints. Donor-site morbidity still main concern after mosaicplasty because it entails harvesting an autograft from otherwise healthy region to be impacted later on weight-bearing damaged site. We describe possible alternative conventional with subchondral bone support harvested iliac crest as osteoperiosteal and covered minced cartilage layer.
