Substance use disorders (SUDs) and anxiety (ADs) are highly comorbid, a co-occurrence linked to worse clinical outcomes than either condition alone. While the neurobiological mechanisms involved in SUDs intensively studied separately, underlying their comorbidity remain an emerging area of interest. This narrative review explores processes this comorbidity, using Research Domain Criteria (RDoC) framework map disruptions positive valence, negative cognitive systems across three stages addiction cycle: binge/intoxication, withdrawal/negative affect, preoccupation/anticipation. Anxiety substance play reciprocal role at each stage addiction, marked by significant psychosocial impairment dysregulation brain. A more thorough understanding neural underpinnings comorbid will contribute tailored effective therapeutic interventions assessments.

Individuals with comorbid posttraumatic stress disorder (PTSD) and substance use (SUD) present a diversity of symptoms. Current interventions show minimal efficacy differences have high attrition. Offering variety treatment options, including virtual ones, ensures access that is appropriate acceptable to individual needs. We conducted single-arm pilot study examine the acceptability feasibility an online intervention based on Internal Family Systems (IFS), called Program for Alleviating Reducing Trauma, Stress, Substance Use (PARTS-SUD). Ten adults (N=10), diagnoses PTSD SUD, were allocated 12 weekly groups 6 counseling sessions. Our pre-specified aims (70% overall acceptability, 75% willingness refer friend), completion), key exploratory clinical outcomes (PTSD symptom severity craving). Participants rated mean score 86% 92% friend, retaining 70% participants at weeks. Furthermore, symptoms reduced by 1.7 points/week (95% CI: -2.45, -0.93, p=0.002) 54% sample achieving minimally important difference in PCL-5 scores. Craving Scale scores 0.25 -0.45, -0.06, p=0.014). An IFS was feasible way provide whole-person people PTSD-SUD within diverse community mental health center setting. An Update on Psychotherapy for the Treatment of PTSD DOI

Posttraumatic stress disorder (PTSD) symptoms are part of the normal response to trauma. Most trauma survivors will recover over time without intervention, but a significant minority develop chronic PTSD, which is unlikely remit intervention. Currently, only two medications, sertraline and paroxetine, approved by U.S. Food Drug Administration treat combination brexpiprazole MDMA-assisted therapy have FDA applications pending. These pharmacotherapy psychotherapy, not recommended as first-line treatments in any published PTSD treatment guidelines. The interventions trauma-focused psychotherapies; Department Veterans Affairs/Department Defense guideline recommends prolonged exposure (PE), cognitive processing (CPT), eye movement desensitization reprocessing, American Psychological Association PE, CPT, therapy, cognitive-behavioral therapy. Although clinical trials psychedelic-assisted psychotherapy incorporated evidence-based psychotherapies, they achieved greater rates than other treatment, there some enthusiasm about combining psychedelic medications with psychotherapies. state-of-the-art psychotherapies briefly reviewed here, including their effects on neurobiological measures.
