Fibroblast activation protein (FAP) is a serine protease selectively expressed in cancer-associated fibroblasts (CAFs), fibrotic tissues, and areas of active tissue remodeling, making it an attractive target for diagnostic imaging across spectrum disease. FAP inhibitors (FAPIs) labeled with PET tracers have rapidly advanced as novel modality broad clinical applications that offers several advantages, including rapid tumor accumulation, low background uptake, high tumor-to-background ratios. In oncology, FAPI has demonstrated excellent performance visualizing wide range malignancies, those glycolytic activity, such pancreatic cancer, cholangiocarcinoma, certain sarcomas. Its sensitivity specificity the stromal component enables improved delineation, staging, response assessment. Additionally, potential to guide theranostic approaches, where same tracer can be therapeutic radionuclides, positions key player precision oncology. The role of fibroblast activation protein inhibitor (FAPI) positron emission tomography/computed tomography (PET/CT) is emerging for the assessment non-oncological diseases, such as inflammatory and infectious even if evidence in literature still its initial phases. Background Total hip arthroplasty (THA) is a well-established surgical procedure that has been extensively validated to alleviate pain, enhance joint function, improve the ability perform daily activities, and overall quality of life. However, this associated with certain complications, among which skeletal muscle fibrosis frequently overlooked but significant complication can lead persistent pain. Currently, there no effective method for diagnosing following total arthroplasty. Case report We 75-year-old male patient who complained left groin pain after revision Serological examinations, X-rays, bone scan results were all normal. during 68 Ga-FAPI PET/CT examination, we observed radiotracer uptake along iliopsoas muscle. This abnormal pattern suggested potential biological activity in specific area. Combined physical was diagnosed fibrosis. Conclusions The presented images indicated an important indicator diagnosis, prospect fibroblast activation protein diagnosis shown application value.
