SUMMARY Glucocorticoids (GCs) are used as anti-inflammatory and immunosuppressive drugs in many immune mediated diseases, but their use sepsis shock is controversial. This caused part by a lack of information regarding the responding cell types GC-regulated genes vivo . We public blood transcriptomic datasets GC-induced query to obtain 2 robust gene expression correlation signatures GC induction, either absence or presence severe inflammation. signature 1 originated from circadian cortisol with biases for NK cells neutrophils. inflammation, mainly monocytes Many upregulated treatment septic burn were also present high-ranking signatures, which pointed direct regulation GC. Today's challenge for precision medicine involves the integration of impact molecular clocks on drug pharmacokinetics, toxicity, and efficacy toward personalized chronotherapy. Meaningful improvements tolerability and/or medications through proper administration timing have been confirmed over past decade immunotherapy chemotherapy against cancer, as well commonly used pharmacological agents in cardiovascular, metabolic, inflammatory, neurological conditions. Experimental human studies recently revealed sexually dimorphic circadian responses. Dedicated randomized clinical trials should now aim to issue recommendations daily medical practice, integrating innovative technologies remote longitudinal monitoring metrics, statistical prediction clock function from single-timepoint biopsies, multiscale biorhythmic mathematical modelling. Importantly, chronofit patients with a robust function, who would benefit most chronotherapy, need be identified. Conversely, nonchronofit could emerging class chronobiotics targeting clock.

Gut microbiota is the largest and most complex microflora in human body, which plays a crucial role health disease. Over past 20 years, bidirectional communication between gut extra-intestinal organs has been extensively studied. A better comprehension of alternative mechanisms for physiological pathophysiological processes could pave way health. Cardiovascular disease (CVD) one common diseases that seriously threatens Although previous studies have shown cardiovascular diseases, such as heart failure, hypertension, coronary atherosclerosis, are closely related to microbiota, limited understanding pathogenesis leads poor effectiveness clinical treatment. Dysregulation inflammation always accounts damaged gastrointestinal function deranged interaction with system. This review focuses on characteristics CVD significance regulation during whole process. In addition, strategies prevent treat through proper its metabolites also discussed.

Shift work is associated with systemic chronic inflammation, impaired host and tumor defense dysregulated immune responses to harmless antigens such as allergens or auto-antigens. Thus, shift workers are at higher risk develop a autoimmune disease circadian disruption sleep impairment seem be the key underlying mechanisms. Presumably, disturbances of sleep-wake cycle also drive skin-specific diseases, but epidemiological experimental evidence so far scarce. This review summarizes effects work, misalignment, poor sleep, effect potential hormonal mediators stress melatonin on skin barrier functions innate adaptive immunity. Human studies well animal models were considered. We will address advantages pitfalls in possible confounders that could diseases adverse lifestyle habits psychosocial influences. Finally, we outline feasible countermeasures may reduce autoimmunity workers, treatment options highlight outstanding questions should addressed future studies.

Abstract Study Objectives Although poor sleep quality is associated with lower CD4+ T cell counts among people living HIV (PLWH), the association between objective metrics and lymphocyte subset unknown. We evaluated polysomnography (PSG) derived subpopulations in a cohort of men HIV. Methods Virally suppressed participating Multicenter AIDS Cohort underwent home overnight PSG. assessed PSG parameters CD8+ CD4+/CD8+ ratio. Results Overall, 289 mean (±SD) age 55.3 ±11.3 years count 730 ±308 cells/mm3 were evaluated. Total time (TST) was significantly but not counts. The circadian clock and its inherent rhythms serve as the foundation for regulating many of body’s vital processes. Recent advances in medicine have leveraged these natural cycles through chronotherapy, practice timing drug administration to align with body's internal clock, enhance treatment outcomes. Endogenous clocks not only dictate physiological but also influence how drugs are absorbed, distributed, metabolized, excreted, thereby affecting both their therapeutic efficacy potential toxicity. When disrupted, resulting imbalance has been linked a range disorders, such cancer cardiovascular diseases, highlighting growing need time-sensitive strategies. Technological breakthroughs, including wearable sensors, digital health platforms, sophisticated machine learning techniques, now allow continuous, real-time monitoring biomarkers, which turn helps optimize regimens. In oncology, example, synchronizing chemotherapy, therapy, immunotherapy anti-inflammatory patient’s cycle shown effectiveness improve tolerability. Moreover, observed sex-based differences responses emphasize importance tailoring individual patient profiles. Emerging research is focusing on directly targeting molecular components, showing treating metabolic oncological conditions. However, fully integrating chronopharmacology into clinical will require development standardized protocols, regulatory backing, large-scale trials that incorporate biomarkers. Ultimately, merging biology strategies promises efficacy, minimize side effects, Future studies must address inter-individual variability, sex differences, misalignment harness chronotherapy’s personalized medicine.

The global rise of autoimmune diseases presents a significant medical challenge, with inadequate treatment options, high morbidity risks, and escalating healthcare costs. While the underlying mechanisms disease development are not fully understood, both genetic predispositions lifestyle factors, particularly sleep, play critical roles. Insomnia circadian rhythm sleep disorders only impair but also disrupt multi-organ interactions by dysregulating sympathetic nervous system activity, altering immune responses, influencing neuroendocrine function. These disruptions can contribute to dysregulation, increasing risk development. To assess impact impaired on developing diseases, population-based retrospective cohort study was conducted using electronic health records from TriNetX US Global Collaborative Network, including 351,366 subjects in each propensity score matched group. Twenty were examined, matching employed reduce bias. Three sensitivity analyses test robustness results. identified significantly increased risks for several associated likely mediated dysregulated neuroimmune autonomic interactions. Specifically, cutaneous lupus erythematosus [hazard ratio (HR) = 2.119; confidence interval (CI) 1.674-2.682; p < 0.0001], rheumatoid arthritis (HR 1.404; CI 1.313-1.501; 0.0001), Sjögren syndrome 1.84; 1.64-2.066; thyroiditis 1.348; 1.246-1.458; 0.0001) showed risks.
