Non-motor symptoms (NMS) in Parkinson's disease (PD) significantly impact quality of life, especially later stages. REM sleep behavior disorder (RBD) affects approximately 42% all PD patients and frequently precedes motor symptoms. RBD is linked to increased rates depression cognitive decline. This study explores how early can predict the development NMS profiles patients, comparing those with without RBD. We identified a unique profile associated These had higher rate impairment (M2 = 326.84, p < 0.001), apathy 332.93, hallucinations 480.05, 0.0001), anxiety (corrected 0.01), impulse control disorders 77.577, autonomic dysfunction (F 251.8, 0001). A survival analysis revealed an association between faster progression impairment. may play critical role stratifying for targeted interventions.

Abstract Despite its high prevalence among dementias, Lewy body dementia (LBD) remains poorly understood with a limited, albeit growing, evidence base. The public‐health burden that LBD imposes is worsened by overlapping pathologies, which contribute to misdiagnosis, and lack of treatments. For this report, we gathered analyzed public‐domain information on advocacy, funding, research outputs, the therapeutic pipeline identify gaps in each these key elements. To further understand current gaps, also conducted interviews leading experts regulatory/governmental agencies, academic research, biopharmaceutical as well funding sources. We identified wide across entire landscape, most critical being research. Many participated workshop discuss prioritization areas view accelerating development improving patient care. This white paper outlines opportunities for bridging major creates framework collaboration endeavor. Lewy body dementia (LBD), the second most common degenerative after Alzheimer's disease, is frequently associated with neuropsychiatric symptoms such as depression, anxiety, and apathy. These may precede cognitive decline, often resulting in misdiagnosis inappropriate treatment. Electroconvulsive therapy (ECT) has emerged a promising option for treatment-resistant depression LBD. This report describes 68-year-old female patient LBD who received multiple ECT sessions persistent severe suicidal ideation. led to marked symptom improvement across several hospitalizations. case underscores diagnostic therapeutic challenges of highlights potential alternative when pharmacotherapy inadequate. Abstract In addition to the core clinical features of fluctuating cognition, visual hallucinations, and parkinsonism, individuals with dementia Lewy bodies (DLB) frequently experience chronic debilitating major depression. Treatment depression in DLB is hampered by a lack available effective therapies standard serotonergic medication for depressive disorder (MDD) typically ineffective. Dysfunction dopaminergic neurotransmission contributing anhedonia loss motivation has been described MDD. The subgenual anterior cingulate cortex (sgACC) important mood regulation symptomatic expression depression, displaying structural, functional metabolic abnormalities To assess synaptic changes DLB, post mortem sgACC tissue from donors without was investigated using high-resolution stimulated emission depletion (STED) microscopy, as well Western dot blotting techniques. STED imaging demonstrated presence α-synuclein within individual terminals sgACC, showing significant positive correlation increased synaptosomal associated protein 25 kDa (SNAP25) volumes depressed cases. A reduction innervation observed cases compared controls ( p < 0.001), but not non-depressed donors, along reduced levels multiple markers receptors. Limited alterations were markers. Our work demonstrates role aetiology DLB. Careful selective targeting systems may be therapeutic option treatment

In recent years, environmental factors have received attention in the pathogenesis of neurodegenerative diseases. Other than genetic factors, identification and modifiable risk may create opportunities to delay onset or slow progression Lewy body disease. Researchers made significant progress understanding over past 30 years. To date, despite increasing number articles assessing for disease, few reviews focused on their role its onset. this review, we reviewed literature investigating relationship between disease several other factors. ABSTRACT Objectives Managing symptoms, notably psychiatric in dementia with Lewy bodies (DLB) is complex, affecting both patients and caregivers. People DLB often react poorly to antipsychotics, limiting treatment options. Although electroconvulsive therapy (ECT)'s potential for acknowledged, evidence scarce owing limited studies. This study investigated ECT's effectiveness safety prodromal antecedent symptoms. Methods retrospective people ( N = 12) mild cognitive impairment (MCI) LB 13), a form of DLB, who underwent ECT symptoms had abnormal findings confirmed using dopamine transporter single‐photon emission computed tomography 123 I‐metaiodobenzylguanidine myocardial scintigraphy. We reviewed these patients' medical records determined the severity psychotic before 1 week after final session Clinical Global Impressions Severity Scale (CGI‐S). Improvement was evaluated approximately CGI (CGI‐I). Additionally, we assessed function ECT, as well any adverse events caused by ECT. Results significantly improved CGI‐S, CGI‐I reports order 60% “very much improved,” 20% “much 16% “minimally 4% “no change.” Parkinsonism (Hoehn Yahr: 1.76 ± 1.2 vs. 1.04 0.7 after, p < 0.001) did (Clinical Dementia Rating, 0.037). Adverse included delirium 24% amnesia patients. not worsen function. Conclusions MCI appears safe effective managing Parkinsonism. Further large‐scale multicenter studies are warranted conclusively establish its safety.
