This manuscript explores the role of pyroptosis, an inflammatory programmed cell death, in pathogenesis two chronic dermatoses, psoriasis and hidradenitis suppurativa (HS). The diseases, though clinically diverse, share common pathogenetic pathways involving unbalanced interaction between adaptive innate immune systems. review focuses on molecular changes psoriatic HS skin, emphasizing activation dendritic cells, secretion interleukins (IL-17, IL-22, TNF-α), involvement inflammasomes, particularly NLRP3. discusses caspases, especially caspase-1, driving pyroptosis highlights family gasdermins (GSDMs) as key players formation pores leading to rupture release proinflammatory signals. study delves into potential therapeutic implications targeting HS, examining existing medications like biologics Janus kinase inhibitors. It also reviews current limitations challenges developing therapies that selectively target pyroptosis. The microbiota based dietary interventions have emerged as an unconventional bacteriotherapeutic approach for the treatment of a plethora pathological conditions including inflammatory bowel disease. potential side effects associated with use probiotics include systemic infections, deleterious metabolic activities, excessive immune stimulation in susceptible individuals and gene transfer. Metabolites resulting from the bacterial fermentation of dietary fibers, such as short-chain fatty acids, especially butyrate, play important roles in maintaining gut health and regulating various biological effects skin. However, butyrate is underutilized due to its unpleasant odor. To circumvent this organoleptic unfavorable property, phenylalanine butyramide (PBA), a precursor, has been synthesized currently available on market. Zusammenfassung Die Erforschung kaum einer anderen Erkrankung hat so grundlegend neue Erkenntnisse über immunologischen Regulationen und gleichzeitig eine derart rasante Entwicklung innovativer Therapien hervorgebracht wie das Studium der Psoriasis. In dieser kurzen Übersichtsarbeit umreißen wir die Grundzüge aktuellen Psoriasisforschung unter besonderer Berücksichtigung pathophysiologischer Vorgänge Meilensteine bei Zulassung verschiedener Biologika niedermolekularer Pharmaka. Nicht zuletzt durch verstehen Zusammenspiel Komponenten des angeborenen adaptiven Immunsystems immer besser. Neue Therapeutika greifen dabei an entscheidenden Punkten in regulatorische Netzwerke ein. Ausgehend vom derzeitigen Kenntnisstand skizzieren einige aus unserer Sicht wesentliche zukünftige Forschungsrichtungen sowie therapeutische klinische Entwicklungen im Bereich Diese reichen von genetischer, epigenetischer, zellulärer immunologischer Mechanismen Untersuchung spezifischer klinischer Formen Psoriasis individueller systemischer Auswirkungen Krankheit ihrer Behandlung bis hin zur Einbeziehung Big Data künstlicher Intelligenz. Ziele sind ein ganzheitliches Verständnis molekularen organismischen gesellschaftlichen Ebene darauf aufbauend individualisierte Präventions‐ Behandlungsstrategien. Trotz beeindruckender Fortschritte muss sowohl kleinen als auch großen Maßstab weiterentwickelt werden, um den Bedürfnissen Behandlern Patienten noch besser gerecht zu werden.
