Extracellular vesicles (EVs) are nano-sized, membranous structures secreted by cells into the extracellular space, have attracted considerable attention in field of biosciences for their role intercellular communication various physiological and pathological processes. Their ubiquitous presence bodily fluids cell-specific characteristics make them promising candidates as biomarkers. Additionally, ability to transport biological therapeutics across different barriers specific target underscores significant translational potential diagnostic therapeutic purposes. Significant progress has been achieved translation EVs research clinical applications, however, challenges persist large-scale production EVs, particularly areas scalable manufacturing, efficient isolation methods, drug loading techniques, advanced characterization technology. This review highlights the key molecular and cellular mechanisms contributing to aging, such as DNA damage, mitochondrial dysfunction, telomere shortening, protein defective autophagy. These biological are involved in various oral health conditions prevalent elderly, including periodontal disease, cancer, xerostomia, dental caries, temporomandibular joint disorders. Exosomes generated by mesenchymal stem cells possess substantial therapeutic potential. exosomes nanosized extracellular vesicles derived from essential intercellular communication tissue homeostasis. The exosome-based therapies proved superior traditional cell-based approaches, due lower immunogenicity, ease of storage, avoidance complications associated with cell transplantation. Osteoarthritis, affecting over 8 million people in the UK, remains a debilitating condition with limited treatment options. Current therapies primarily address symptoms and can exacerbate joint damage time. Developing disease-modifying drugs that alleviate inflammation promote regeneration is crucial for long-term patient benefit. This study investigates potential of exosome-like nano-vesicles isolated from grapefruit juice (GEVs) as novel therapeutic approach osteoarthritis. GEVs possess regenerative properties present promising avenue clinical translation. In this study, were characterized terms protein quantification, size, morphology. vitro studies demonstrated safety efficacy GEVs, showing an enhancement human chondrocyte migratory activity 13%. exhibited dual mechanism action, reducing oxidative stress while promoting cellular regeneration. Specifically, they reduced expression COX2 PTGS2, markers associated pain sensitization, enhanced antioxidant genes SD2 GPX osteoarthritic-like chondrocytes. Additionally, downregulated ADAMTS-5 hypertrophic COL10 upregulating chondrogenic ACAN, COL2, SOX9. research signifies significant advancement osteoarthritis therapy, offering natural, safe, cost-effective option long-lasting benefits. Clinical translation GEV therapy holds promise improving outcomes burden on healthcare systems.

ABSTRACT Sepsis‐induced cardiomyopathy (SICM) is a complex and fatal manifestation of sepsis, characterised by myocardial dysfunction that exacerbates the clinical prognosis in septic patients. While pathophysiology SICM remains incompletely understood, emerging evidence highlights multifaceted functions exosomes, small membrane‐bound extracellular vesicles, mediating inflammatory responses cardiac involved this condition.
