This study aims to investigate alterations in immune cell counts within preovulatory follicles of patients with poor ovarian response (POR) during assisted reproductive technology (ART), classified according the POSEIDON criteria. single-centre cross-sectional included 543 women undergoing IVF/ICSI treatment, selected based on specific inclusion and exclusion criteria: 292 normal 251 response. Follicular fluid (FF) was collected day oocyte retrieval analysed by flow cytometry determine proportions macrophages (Mφs), M1 M2 Mφs, T cells (CD4 CD8 cells), dendritic (DCs), including type 1 conventional (cDC1) 2 (cDC2), neutrophils. Multivariable logistic regression assessed relationship between POR, Pearson correlation determined associations number retrieved oocytes, receiver operating characteristic (ROC) curves evaluated predictive power for POR. Immune accounted 52.57% (±23.90%) total population follicular microenvironment, which approximately equal that granulosa cells, Mφs being most abundant, followed sequentially DCs, In overall infiltration microenvironment decreased, whereas polarization increased. increased, a decrease CD4/CD8 ratio. Both cDC1 cDC2 were significantly elevated. Moreover, multivariable revealed macrophage count, CD4 count independent predictors Notably, showed largest area under ROC curve, suggesting its strong potential as biomarker predicting The proportion altered These findings suggest dynamics may play crucial role determining prognosis, indicating targeted immunomodulatory strategies could be considered future therapeutic approaches.

Polycystic ovary syndrome (PCOS) is a common endocrine disorder among females of reproductive age with heterogeneous prevalence. It well known that female competence depends on the dynamic regulation hypothalamic–pituitary–gonadal (HPG) axis; therefore, disruption this highly regulated system leads to fertility problems. Among disruptors, both oxidative stress and inflammation contribute an increased LH-FSH ratio consequent hyperandrogenism. Shifts in bidirectional interplay between neuroendocrine oxidative/inflammatory homeostasis result accumulation reactive oxygen/nitrogen species inflammatory markers as alterations antioxidant defense mechanisms. Evidence shows lifestyle changes, including regular physical exercise, are recognized most effective first-line management reduce severity PCOS symptoms. Reproduction consists of sequential inflammation-like events, primarily within the endometrium, from ovulation to embryo implantation, decidualization and delivery. During reproductive cycle, endometrium repeatedly undergoes cyclic periods proliferation, differentiation, tissue breakdown repair without scarring. Owing their phagocytic activity, macrophages, key players in innate immunity, are thought play crucial roles endometrium. Endometrial macrophages actively participate various stages remodeling, particularly during pregnancy establishment. Traditionally considered simple bystanders that clear debris prevent autoimmune responses homeostasis, now recognized as main actors with broad functional plasticity allows them fine tune balance between pro- anti-inflammatory inflammation, remodeling repair. Homeostatic is determined by sum mediators produced two distinctly polarized macrophage subpopulations. The biased polarization tissue-resident may contribute pathogenesis diseases, such inflammation cancer. Thus, understanding how endometrial homeostasis for deciphering underlying mechanisms disorders. Nanomedicines using extracellular vesicles, nanoparticles noncoding RNAs have recently been applied modulate alleviate disease phenotypes. Despite these advances, functions under physiological pathophysiological conditions remain poorly understood, which complicates development targeted therapies. Here we update current homeostatic function putative contribution dysfunction disorders women, along innovative molecular therapeutics resolve this issue.

Increasing studies focused on the efficacy of acupuncture pregnancy outcomes in patients with polycystic ovary syndrome (PCOS) undergoing vitro fertilization-embryo transfer (IVF-ET). However, debatable conclusions have been drawn from different randomized controlled trials (RCTs), which might be related to doses acupuncture. To evaluate whether has a dose-dependent effect PCOS IVF-ET systematically reviewing. Seven electronic databases were searched inception October 10th, 2024. The Cochrane Collaboration's tool ROB 2.0 (ROB 2.0) provided an assessment for risk bias. dose was extracted, then categorized into high, medium, and low according scoring system results, evidence assessed by Slavin's qualitative best-evidence synthesis approach rigours methodological way. Clinical rate (CPR) regarded as primary outcome. A total 953 subjects met eligibility criteria 12 RCTs included, among two dose, four medium six high dose. overall quality included low, 50.00% (6/12) risk, 16.67% (2/12) some concerns, 33.33% (4/12) risk. Comparing consistent high-dose result high-quality provides strong positive correlation between outcomes. trend indicates that higher yield better IVF-ET. Few cohort studies have evaluated the long-term impacts of ambient fine particulate matter (PM2.5) and its components on reproductive hormone levels in female adults. We conducted a 15-year retrospective study Taiwan between 2003 2017. The two-year average concentrations PM2.5 components, including sulfate (SO42-), nitrate (NO3-), ammonium (NH4+), organic (OM), black carbon (BC), were assessed at each participant's addresses. Linear mixed models used to examine associations with hormones, follicle-stimulating (FSH), luteinizing (LH), testosterone (T), estradiol (E2), prolactin (PRL). Stratified analyses identify vulnerable populations. 17,152 adults included. Each interquartile range (IQR: 3.545 μg/m3) increase was associated 0.585 mIU/mL [95% confidence interval (CI): 0.190-0.980] FSH levels. Among five PM2.5, BC had strongest positive association [each IQR (0.272 0.863 (95% CI: 0.476-1.250) levels], followed by OM, SO42-, NH4+. Similar found for LH, 0.483 0.225-0.742) 0.684 0.431-0.938) LH per BC, respectively. pollutants marginally decreased E2 increased PRL Non-linear FSH, E2, observed. These also positively T among young Post-menopausal women more susceptible chronic components. Our highlighted adverse exposure hormonal homeostasis, revealing biological mechanism air pollution-reproductive health females. Implementing stringent control pollution can benefit adults, even moderately polluted regions.
