A cutting-edge approach in cell-based immunotherapy for combating resistant cancer involves genetically engineered chimeric antigen receptor T (CAR-T) lymphocytes. In recent years, these therapies have demonstrated effectiveness, leading to their commercialization and clinical application against certain types of cancer. However, CAR-T therapy faces limitations, such as the immunosuppressive tumour microenvironment (TME) that can render cells ineffective, adverse side effects therapy, including cytokine release syndrome (CRS). Extracellular vesicles (EVs) are a diverse group membrane-bound particles released into extracellular environment by virtually all cell types. They essential intercellular communication, transferring cargoes proteins, lipids, various RNAs, DNA fragments target cells, traversing biological barriers both locally systemically. EVs play roles numerous physiological processes, with those from immune non-immune capable modulating system through activation or suppression. Leveraging this capability enhance could represent significant advancement overcoming its current limitations. This review examines landscape explores potential role augmenting therapeutic efficacy.

Immunotherapy, a rapidly evolving field oncology, is type cancer treatment relying on body's own immune system to fight mutated cells. Recently, significance immunotherapeutic resistance has been increasingly acknowledged owing heightened prevalence and its commonly advanced stage upon diagnosis. However, complexity heterogeneity tumor cells pose challenges immunotherapy, role EVs remains unclear. Recent studies focused as heterogeneous groups nanoparticles intercellular within microenvironment (TME). EVs, which include exosomes, shed microvesicles, while apoptotic bodies carry diverse range molecular cargo, including proteins, nucleic acids, lipids, other bioactive molecules. Non-coding RNAs (ncRNAs) are crucial molecules that do not encode proteins but play roles in regulating various biological processes. Recent research highlights ncRNAs only control gene expression within cells also facilitate intercellular communication via exosomes and other carriers. This function is vital the tumor microenvironment (TME). Our review covers structure functions of different ncRNAs, such as microRNAs (miRNAs), long non-coding (lncRNAs), circular (circRNAs). We examine how these influence initiation progression. Additionally, we explore their role promoting growth or immune evasion by modulating TME. The potential using therapeutic targets biomarkers for clinical use discussed. As our understanding grows, development new therapies based on anticipated to offer improved treatment options cancer patients.

Background Cancer-associated fibroblasts (CAFs) and hepatocellular carcinoma (HCC) cells interact to promote HCC progression, but the underlying mechanisms remain unclear. Serpin family E member 1 (SERPINE1) has conflicting roles in HCC, microRNAs (miRNAs) are known regulate tumor progression through intercellular communication. Therefore, we investigated potential involvement of miRNA/SERPINE1 axis crosstalk between CAFs cells. Methods In this study, candidate miRNAs targeting SERPINE1 3′ UTR were predicted using multiple miRNA databases.
